Objective: To analyze the clinical effect of percutaneous pedicle screw fixation in the treatment of adolescent thoracolumbar fractures without neurological injury.Methods: retrospective analysis 20 patients of adolescent thoracolumbar fractures without neurological injury from January 2006 to January 2016 which were divited into two groups:group A(Percutaneous group)and group B(open group).Group A: 10 cases were treated by percutaneous pedicle screw fixation.Group B:10 cases were treated by posterior open reduction and internal fixation.throngh compare and observe the change before and after the operation of vertebral height restoration,kyphotic Cobb angle,comparison of VAS score difference before and after surgery,with injured vertebra.And compared the Index of surgical trauma such as the surgical blood loss,surgical draining loss,surgical time,hospital stay to analyze the clinical effect of percutaneous pedicle screw fixation.Results: There were significant differences between the Percutaneous group and the open group in the surgical blood loss,surgical draining loss,surgical time,VAS pain score,Fluoroscopy times and radiation(P<0.05);There were no significant difference in the bed stay,vertebral height,Cobb's angle,spinal canal encroachment correcting both and the accuracy of transpedicular screw placement between the two groups(P>0.05).Conclusions:1.Adolescent spinal fractures generally do not appear to damage the intervertebral disc,it has the possibility of spinal injury of no obvious vertebral fracture of the spine.Multi segmental fractures and fracture dislocations are more common in adolescents with spinal fractures.If the vertebral endplate is damaged,it can easily lead to spinal deformity.2.Percutaneous pedicle screw fixation is not as effective as conventional open surgery.Moreover,the percutaneous group had less trauma and faster functional recovery than the open group.And the accuracy of the two groups was similar.It is is a safe and effective therapeutic method,Worthing of further clinical promotion and Application.At the same time,the radiation in clinical was very large,which must be concerned.3.For patients with severe vertebral burst fracture and spinal cord injury,it is more reliable to choose the conventional open and decompression operation.
