Objective: To investigate the clinical effect of percutaneous pedicle screw fixation in treatment of thoracolumbar vertebral compression fractures. Methods: Between October 2014 to October 2015,64 cases of simple thoracolumbar compression fracture patients in XX University Affiliated XX Hospital were divided into two groups,a group for the observation group(n = 34),another group as reference group(n = 30).The observation group using the minimally invasive percutaneous pedicle screw internal fixation, while the reference group was treated with conventional posterior median longitudinal incision of open surgery.Statisticing and analyzing related dates after the operation,to compare the clinical efficacy of the two surgical treatment of thoracolumbar vertebral compression fractures.Do X-ray image data measurement for X-rays perioperatively all involved in the study, study, including anterior height of vertebral body, the kyphosis angle(Cobb angle)and the sagittal index, and make a record.Records postoperative pain degree and serum C reaction protein changes of patients in different time, pain degree could been said using specific number of visual analogue scale(visual analogue scale,VAS).Detailed records of patients with operation time, intraoperative bleeding volume, postoperative drainage volume, hospitalization time and postoperative complications(mainly Failed back Surgery Syndrome)occurs.Use spss21.0 to comparative analysis of two groups of patients with operation time, intraoperative bleeding volume, postoperative drainage volume, length of hospital stay, fracture healing time,The height of the injured vertebra,Cobb angle, sagittal index, serum C-reactive protein,VAS and postoperative failed back surgery syndrome(FBSS)situation and so on.Operation time, intraoperative bleeding volume, postoperative drainage volume, hospitalization days,VAS and serum C-reactive protein are independent samples t test.Imaging indexes of the height of the injured vertebra,Cobb angle and sagittal position index were analyzed by paired t test and independent samples test. Results: The observation group compared with the reference group, shorten the average time of operation, intraoperative bleeding volume and postoperative drainage volume significantly reduced, length of hospital stay was significantly reduced(P <0.05);Between the two groups of patients, image indexex like the height of the injured vertebra,Cobb angle and sagittal niche index had no significant difference perioperatively(P > 0.05);The observation group has more obvious pain relief compared with the reference group(P<0.05),C-reactive protein of the reference group increased more significantly compared with the observation group(P<0.05);patients in reference group have a high FBSS rate.Conclusions:1.Minimally invasive percutaneous pedicle screw internal fixation compared with traditional open surgery, image indexes like height of the injured vertebra,Cobb angle, and sagittal niche index had no significant difference perioperatively, indicate that minimally invasive pedicle screw internal fixation did not affect surgical correction effect.2.The minimally invasive percutaneous pedicle screw internal fixation compared with traditional open surgery, shorten the average time of operation, intraoperative bleeding volume and postoperative drainage volume decreased significantly, significantly shorten the hospitalization time, postoperative pain relief obviously, postoperative C-reactive protein increased a lesser extent.Minimally invasive percutaneous pedicle screw internal fixation for the treatment of the patients caused smaller wound, reduce the pain of patients, more conducive to the healing of fractures.3.Minimally invasive percutaneous pedicle screw fixation surgery compared withtraditional open surgery, postoperative pain relief was more obvious,C-reaction protein increased a lesser extent, there may be a correlation between them.
