Objective:To investigate the safety and efficacy of reducing blood loss in total hip arthroplasty through injecting the tranexamic acid with closed drainage into intra-articular.Methods: According to a retrospective study of the requirements of unilateral total hip arthroplasty in Affiliated Hospital of Shandong University of Traditional Chinese Medicine from March 2015 to December 2016,which includes 60 patients in the test group,with the local application of tranexamic acid;and 60 cases in the control group,without the use of tranexamic acid,we could conclude a bright result.The two groups were all placed into the joint cavity and a drainage tube which use the autologous blood transfusion.Then we should analyze the differences between the two groups of patients in the aspect of postoperative drainage volume,blood loss,blood transfusion,Harris hip score and complications.Results:Comparing the topical application of tranexamic acid group and the control group,there is no significant difference in gender,preoperative age and the body mass index(P>0.05).However the general quantity of the postoperative drainage volume,postoperative 3 days dominant blood loss,total blood loss,blood transfusion amount and so on are all decreased,and the difference was statistically significant(P<0.05),whereas there is no significant difference in the amount of hidden blood loss(P>0.05).For the postoperative complications,the differences are not statistically significant in DVT,lower extremity lymph edema,ecchymosis,pulmonary infection and so on(P>0.05).About the Harris score of hip joint,there is no significant difference between the two groups(P>0.05),and the scores are improved after three months.Conclusion: In the total hip replacement,injecting the tranexamic acid with short-term occlusion of the drainage into the tubeintra-articular can reduce the postoperative of the drainage,total blood loss and allogeneic blood use,at the same time it could not increase the incidence of complications,even the the recovery of the hip function.
