| Purpose: By studying the factors influencing the delay in the treatment of breast cancer patients, and understanding the occurrence and impacts of that, guidance can be provided for the early diagnosis and treatment of breast cancer patients.Methods: A total of 539 patients with breast cancer who are confirmed for the first time and have undergone surgery in the Third Affiliated Hospital of Kunming Medical University from January 2016 to January 2017 are collected. The required statistical data is collected from the case system. Statistical variables include: demographic information and data of clinical characteristics. The demographic information includes: age, nation, type of residence and work, marital status, medical insurance,ect. The data of clinical characteristics includes: the type of patients; first symptom(including breast and non-breast lumps), menstrual history, breastfeeding,location of tumor, examination results of pathology, size of tumor, situation of lymphatic ’and distant metastasis, the time of patients’ treatment delay,family history of malignant tumor and breast cancer,and so on. The factors and impacts of treatment delay on breast cancer patients is analyzed by chi-square test and binary logistic regression. The definition of treatment delay is that the interval from breast patients’initial awareness of abnormal symptoms to the diagnosis of the Third Affiliated Hospital of Kunming Medical University is greater than or equal to 3 months (90d).Results: Among the 539 patients, the age of onset was between 25 and 79 years, and the median age was 48 years. The time of medical treatment ranges from 1d to 11880d,and 90d is the median time. The definition of treatment delay is that the interval from breast patients’ initial awareness of abnormal symptoms to the diagnosis of the Third Affiliated Hospital of Kunming Medical University is greater than 90d. The number of treatment delay is 287, accounting for 53.25% of the total number of objects.Statistical difference is existed between the working statue of breast cancer patients(P = 0.003), family history of malignancy (P = 0.043), size of patients’ tumor (P =0.000), distant metastasis (P = 0.036), the size of invasive carcinoma patients,tumor(P = 0.000), distant metastasis (P = 0.004), TNM staging (P = 0.001) and treatment delay. While statistical significance is non-existed between age, nation, type of residence, marital status and treatment delay as well as between patients’ first symptom (including breast and non-breast lumps), menstrual history, breastfeeding,location of tumor, menopause, family history of breast cancer, examination results of pathology, immunohistochemical type (ER, PR, HER2, Ki-67), molecular classification (including LuminalA type, LuminalB type, HER2 overexpression type and triple negative breast cancer), situation of lymphatic metastasis, the size of intraductal carcinoma patients’ tumor and treatment delay.Conclusion: At present, the situation of treatment delay of breast cancer patients is serious. Its factors include the working type and family history of malignant tumor.The negative results of treatment delay are tumor growth of breast cancer patients,increased distant metastasis and higher TNM staging. The longer the delay of medical treatment, the greater the probability of occurrence of advanced breast cancer. Early diagnosis and treatment should be done for patients,thus the occurrence of treatment delay will be reduced. |
