Objective:Analysissing the relevant datas in perioperative period of ederly patients with ACST by common bile stones is for the aim to investigate the risk factors,to provide clues for the operation and clinical diagnosis and treatment so as to prevent the high risk and create the conditions for timely therapy.Methods:To review 63 patients which had been hospitalized in hepatopancreatobiliary surgery department of our hospital in May 2013 to December 2016 and had been diagnosed ACST caused by common bile duct stones,that all 63 patients had performed urgent surgical drainage therpy.Collectting complete clinical hospital datas of the 63 patients and combining with related research and clinical practice is to pick out some possible related clinical factors for statistical treatment.Nonparametic test is used to analysis univariant factors and unconditioned backward Logistic regression method is used to analysis multivariant factors,aiming to identify the related risk factors of ACST by common bile duct stones in ederly patients.Results:In the 63 patients,8 patients that 5 older men and 3 elderly women,had died for the multiple organ failure.Analysissing the 25 factors which were picked out from the clinical data of the 63 cases by univariant analysis,we found that age?75,shock,mental nerve symptoms,coexist chronic disease,previous history of biliary tract surgery,temperature ?39? or ?36?,Neut%?90%,ALB<30g/L,PLT<70×109/L,high TBil level,high DBil level,high ALT level,high AST level,high CRP level,timing of surgery ?8h,anesthesia time ? 150min and transferred to ICU after operation,a total of 17 risk factors whose P value is less than 0.05.By multivariantly analsissing the 17 factors,only age ? 75,shock,mental nerve symptoms,Neut%? 90%,PLT<70×109/L,anesthesia time ?150min and transferred to ICU after operation were the independent risk factors for death of ACST by common bile duct stones in ederly patients.Conclusion:The results of univariant analysis:age?75,shock,mental nerve symptoms,coexist chronic disease,previous history of biliary tract surgery,temperature?39? or ?36?,Neut%?90%,ALB<30g/L,PLT<70×109/L,high TBil level,high DBil level,high ALT level,high AST level,high CRP level,timing of surgery=8h,anesthesia time ?150min and transferred to ICU after operation.The results of multivariant analysis:age ?75,shock,mental nerve symptoms,Neut%?90%,PLT<70×109/L,anesthesia time?150min and transferred to ICU after operation.Using the related risk factors of the disease to improve the early diagnosis and therapy in the actual clinical work,so as to reduce the mortality and improve prognosis of the disease.
