Objective: To study the clinical characteristics of children with HSP,to analyze the relationship between HSP and Epstein-Barr virus infection,to provides a feasibility basis for diagnosis and treatment in the future.Methods: The clinical data and the load of EBV children with HSP who were in Department of Pediatrics, first Affiliated Hospital of Kunming Medical University were collected from September,2014 to January,2016.Then a restrospective study was performed, the children were divide into four groups: simplex purpura, abdominal-related, kidney-related,joint-related, complex purpura. According to the clinical characteristics,the age of onset, gender, season, EBV load, time of hospital stays and relationship between the clinical characteristics and EBV-DNA copies were studied and analysed. According to "Chinses twenty-four solar terms",the spring , summer, autumn and winter were classified.ResultsTThere were 147 boys and 101 girls in 248 children with HSP.The ages were from 2 to 16 years old,the mean age was 8.89±2.89 years old.There were 4 individuals in the group of toddler's age,58 individuals in the group of preschool age, 107 individuals in the group of school age,79 individuals in the group of adolescence. For the seasons, there were 76(30.6%) individuals in spring,36(14.5%) individuals in summer,56(22.6%) individuals in autumn,79 (32.3%) individuals in winter. The disease often happen in spring and winter(P< 0.05).There were 35(14.1%) individuals in the group of simplex purpura,37 (14.9) individuals in the group of abdominal-related purpura,74 (29.8%) individuals in the group of kidney-related purpura,36 (14.5%) individuals in the group of joint-related purpura,66 (26.7%) individuals in the group of complex purpura.There were 105 individuals who were infected by EBV,and the positive rate was 42%.The range of virus load was 4.08×102 to 8.6×105 copy/ml,51(48.6%) individuals was between 4.O×102to 4.0×103copy/ml,45(42.9%) individuals was between 4.0×103to 4.0×104copy/ml, 8(7.6%)individuals was between 4.0×104 to 4.0×105copy/ml, 1(0.9%) individuals was between 4.0×105to 4.0×106copy/ml, the range of virus load was more between 4.0×102 to 4.0×103copy/ml. There were 14 (13.3%)individuals infected by EBV in the group of simplex purpura,16 (15.2%)individuals in the group of abdominal-related purpura,28(26.6%)individuals in the group of kidney-related purpura,17(16.2%) individuals in the group of joint-related purpura,30 (28.7%) individuals in the group of complex purpura. No differences among the different types were in the infection rate. The hospital admitted days were from 3 to 43 days,the mean was 10.91 ±6.64. The hospital admitted days in children with EBV infection more longer than in children without EBV infection(17.12 vs 12.96).Conclusion: The morbidity of HSP in male children is higher than in female children, and often happen in spring and winter. School children have higher incidence. Hospitalized children with HSP have a high positive rate of EBV infection, and the EBV infection would increase the length of stay in the hospital.
