Objective: analysis of ICU(ICU)inpatients with endotoxin,blood amylase,lipase,gastric secretion type immunoglobulin protein A(SIgA),pepsinogen I(PGI),pepsinogen II(PGII),pepsinogen I/II(PGR)and acute gastrointestinal injury(AGI)and its correlation value to the diagnosis of AGI.Methods: 160 patients with acute gastrointestinal injury admitted from October 2015 to December 2016 in Hospital of Wuzhou Red Cross Society were divided into AGI group(n =83)and non AGI group(n=77).Comparison of two groups of patients with endotoxin,serum amylase,lipase,SIgA,I,PG,PG of gastric juice II,PGR index,and through the analysis of operating characteristic curve(ROC)evaluation of endotoxin,blood amylase,lipase,SIgA,I,PG,PG of gastric juice II,PGR index in the diagnosis of acute gastrointestinal injury value price.Results: compared with non AGI group(Z=1.33,P=0.18),there was no significant difference between AGI group(P > 0.05).AGI group of serum amylase and lipase levels higher than those in non AGI group(Z=7.19,P<0.05;Z=5.18,P<0.05),SIgA of gastric juice levels lower than non AGI group(Z=1.98,P<0.05),the difference was statistically significant(P < 0.05).The serum PG I and II levels in AGI group were higher than those in non AGI group(Z=3.27,P<0.05;Z=4.69,P<0.05),and PGR level was lower than that in non AGI group(Z=3.24,P<0.05),and the difference was statistically significant(P <0.05).The sensitivity of serum amylase in >187U/L diagnosis of AGI was 73.5%,the specificity was 84.4%,the area under the ROC curve was 0.83,95%CI(0.76,0.88),a higher diagnostic value;sensitivity of serum lipase >93U/L in the diagnosis of AGI was 57.8%,the specificity was 89.6%,the area under the ROC curve was 0.74,95% CI(0.66,0.81),with moderate diagnostic value;sensitivity of gastric SIgA < 1.8g/L diagnosis of AGI was 74.7%,the specificity was 46.8%,the area under the ROC curve was 0.591,95% CI(0.51,0.69),the diagnostic value is low.The sensitivity and specificity of PGI>130.3 ng/ml in the diagnosis of AGI was 85.71%,and the area under the ROC curve was(0.57,0.72),with a median value of 0.65(CI).The sensitivity and specificity of PG II>23.4ng/ml in the diagnosis of AGI was 61%,and the area under the ROC curve was(0.64,0.78),with a median value of 0.72(CI).The sensitivity and specificity of PGR<2.9 in the diagnosis of AGI was 70.13%,and the area under the ROC curve was 0.65,with a median value of 95% CI(0.57,0.72).Serum amylase,lipase and PG I have high specificity,which can be used as an indicator to identify acute gastrointestinal injury.Conclusion: serum amylase,lipase,gastric juice SIgA,PG I,PG II,PGR has a certain value in the diagnosis of acute gastrointestinal injury in severe patients.At the same time,serum amylase,lipase,PG I has a certain value in differential diagnosis of acute gastrointestinal injury in severe patients.
