| Objective:To explore the effect of cardiopulmonary rehabilitation nursing with kinesitherapy as the central regimen on cardio-pulmonary function,exercise performance and quality of life of inpatients after coronary artery bypass grafting.On the basis of conventional nursing,the patients in the rehabilitation group were given systematic and standardized rehabilitation guidance.Comparing the postoperative cardio-pulmonary function and exercise parameters,as well as the results of the SF-36 health questionnaire and so on,to assess the intervention effectiveness of cardiopulmonary rehabilitation nursing and to provide a theoretical basis for optimizing the nosocomial rehabilitation nursing program.Methods:In this study,we enrolled 100 patients who received coronary artery bypass grafting surgery in the department of surgery of the Fuwai Hospital,Chinese Academy of Medical Sciences from July 2016 to February 2017.Participants were randomly assigned into rehabilitation group(50 cases)and control group(50 cases).The two groups were both administrated with conventional care,while the rehabilitation group was also given the cardiopulmonary rehabilitation nursing which included exercise therapy,respiratory training,patient education and psychological consultation,with kinesitherapy as the central regimen.Before and after the surgery,the SF-36 health questionnaire was conducted to evaluate the quality of life.Five days after surgery,the ultrasonic examination,6MWT and cardiopulmonary exercise testing were conducted to evaluate the patients' cardio-pulmonary function.The muscle strength,balance ability and flexibility parameters were conducted to evaluate the exercise performance of patients.Results:1.There were no significant differences between rehabilitation group and control group in the general demographic information and clinical data before surgery(P>0.05).2.The cardio-pulmonary function:? At baseline,there no statistical significance(P>0.05)in LVED and LVEF between two groups.After the intervention,the LVED became more small and LVEF had improved further.The LVED in the rehabilitation group significantly shrink than those in the control group(P<0.05).? After the intervention,the distance of 6MWT has no statistical significance(P>0.05)between two groups,but the VO2max of rehabilitation group in CPET is larger than control group(P<0.05).3.The exercise performance:? At baseline and after intervention,there were no statistical significance(P>0.05)in the power of gripping between two groups,but the time of sit-to-stand was shorter than the control group.? After intervention,there were no statistical significance(P>0.05)in balance tests between two groups.?After intervention,there were no statistical significance(P>0.05)in flexibility between two groups.4.The quality of life:? At baseline,there were no statistical significance(P>0.05)in the scores of quality questionnaires in every dimension.? After intervention,the scores of all dimensions had improved except GH than pre-operation.The score of MH in rehabilitation group is higher than control group and showed significantly difference(P<0.05).Conclusions:1.Cardiopulmonary rehabilitation nursing can effectively improve the cardio-pulmonary function and aerobic endurance on the inpatients after CABG.2.Cardiopulmonary rehabilitation nursing has some effect in improving exercise performance on the inpatients after CABG.3.Cardiopulmonary rehabilitation nursing can effectively improve the MH scores on the inpatients after CABG.Through one-week cardiopulmonary rehabilitation nursing with kinesitherapy as the central regimen on the inpatients after CABG,the cardio-pulmonary function had improved and the functional restriction which caused by mental problems also get better.The effect of cardiopulmonary rehabilitation nursing is better than conventional nursing,so it is worth popularizing and applying on clinic. |
