Clinical Analysis Of Hemorrhage After Pancreaticoduodenectomy

Objective To investigate the case,prevention and treatment of hemorrhage after pancreaticoduodenectomy(PD).Method The clinical data of 182 patients with 2012.01-2016.12 underwent PD in our hospital were retrospectively analyzed.According to the international collaborative group of pancreatic surgery(ISGPS)on the diagnosis standard of postoperative bleeding,Analysis of preoperative,intraoperative and postoperative related factors in patients with bieeding.To analyze the risk factors of postoperative bleeding,summarize the treatment experience of patients with bleeding after operation,master the prevention and treatment of bleeding.Result Postoperative hemorrhage in 14 cases,the incidence rate of 7.7%,of which 5 cases of gastrointestinal bleeding and 9 cases of abdominal hemorrhage,3 patients bleeded within 24 hours and 11 occured 24 hours later.There were 5 cases of conservative treatment,interventional treatment in 4 cases,surgical treatment in 5 cases,all patients with bleeding were successful hemostasis,no death.Conclusion PD is complicated and diffcult step of surgery,after pancreaticoduodenectomy hemorrhage is one of the most dangerous complication,It has a close relationship with pancreatic leakage.Attention to sentinel bleeding,reduce the occurrence of servere bleeding.PPH in the event of a timely and effective treatment,We can through conservative treatment for mild bleeding,for patients with severe bleeding we should actively intervene or surgical treatment to reduce the mortality of patients.
