| Objective: To analyze and summarize the risk factor of postoperative bleeding afterpancreaticoduodenectomy(PD).search for new method of diagnose and treatmentMethods: The clinical data of132patients receiving PD in first affiliated hospital ofxinjiang medical university from May2005to May2011were analyzed retrospectively.Results: in132patient,73male and59female in this study group,average age was33-82,age was65and above was45,Han people were102,minorities were30,Theincidence of postoperative bleeding was20.5%.postoperative hemorrhage(alimentary tractand abdominal) occurred in27case,alimentary hemorrhage occurred in12case(9.1%),abdominal and alimentary hemorrhage occurred in ninecase(6.8%),abdominal hemorrhage occurred in six case (4.5%), analytic result indicates:intraoperative blood loss,intraoperative blood transfusion,intra-abdominalinfection,pancreatic fistula,biliary fistula and fistula of pancreas-bowels anastomoticstoma were significantly associated with postoperative bleeding after PD(P<0.05).Multivariate analysis, using logistic regression, identified3variables as independentfactors associated with the occurrence of bleeding, operative blood loss(OR=25.759),pancreatic fistula(OR=11.366) and intra-abdominalinfection(OR=4.500).conclusion: operative blood loss,pancreatic fistula,andintra-abdominal infection were independent risk factor of preoperative hemorrhage ofpancreaticoduodenectomy,thus reasonable perioperative management and spetialazieoperation team enough to decrease incidence and mortality of postoperativehemorrhage(abdominal cavity and enteron) effectively. |
