The Study On Traditional Chinese Combined With Western Medicine Clinical Nursing Pathway For Acute Heart Failure Based On The Concept Of Heart Failure Unit Building

Objective Based on heart failure unit concept framework,design of traditional Chinese combined with western medicine clinical nursing pathway for acute heart failure,and to explore the feasibility and effect of the path,to build a quick and effective treatment for acute heart failure patterns provide indeed feasible scheme.Methods This study is divided into two parts.The first part:formulated by reference to the ministry of health in 2009 acute left heart failure clinical pathway(2009 edition),and the state administration of traditional Chinese medicine department of heart failure disease clinical pathway of traditional Chinese medicine in 2011,combined with the existing clinical rescue plan,literature research,expert consultation and so on to develop the acute heart failure combined with traditional Chinese and western medicine clinical nursing pathway,which divided into patients and medical version,using the check list table form.After the approval from the hospital,in the full propaganda and training,perfecting the organization system,on the basis of the path of clinical applications.The second part:adoption of history study.Choice from November 2015 to February 2016 set Tongde hospital of Zhejiang province of 59 cases with acute heart failure patients admitted in the emergency department for sifting,met inclusion exclusion standard the medical records of 41 for information gathering,as control group.Select meets inclusion and exclusion criteria from November 2016 to February 2017 in the hospital emergency department treatment of 49 cases of patients with acute heart failure in heart failure clinical path management unit,as the intervention group.Compared to two groups of patients with first aid,emergency response,emergency expenses efficiency,characteristic index of traditional Chinese medicine and nursing job satisfaction,using the data of MS Excel 365 and drawing,the IBM SPSS 22.0 for data analysis.Results 1.Set the text of traditional Chinese combined with western medicine clinical nursing pathway and implementation process for acute heart failure.Text includes medical version?patient version and clinical nursing path variation form.2.Two groups of cases have no obvious differences in gender,age,structure of basic diseases,the two groups was comparable(P>0.05).Path group preview triage accuracy 93.6%higher than review group 93.6%,both are statistically significant(P<0.05).Preview triage time of review group is 3.15±1.1 seconds shortened to path group 2.27±0.8 seconds,which is statistically significant(P<0.01).Time-to-Treatment window by review group 27.18±8.6 seconds shortened to path group 20.27±7.8 seconds,which is statistically significant(P<0.01).The total cost of path group in intensive care fo1324.60±372.49Yuan,less than review group was 1760.97±629.13 yuan,both is statistically significant(P<0.05).After the path group patients analysis of curative effect of heart failure before and after the clinical nursing pathway management,effect of heart failure scores and heart failure symptom curative effect of traditional Chinese medicine were statistically significant(P<0.05).Satisfaction of nursing work,review group is higher than path group,the difference was statistically significant(P<0.01).Conclusion 1.Developed traditional Chinese combined with western medicine clinical nursing path for acute left heart failure;2.Combined traditional Chinese and western medicine clinical nursing path for acute left heart failure,can significantly improve the efficiency of emergency,effectively shorten the rescue time window,ensure the quality of first aid of patients;3.The implementation of traditional Chinese combined with western medicine clinical nursing path,reduce the cost of first aid,medical costs,has received the good economic benefit;4.The implementation of acute heart failure of Chinese and western medicine combined with clinical nursing path,can guarantee the quality of acute heart failure in first aid.5.The implementation of acute heart failure of traditional Chinese combined with western medicine clinical nursing pathway,increased participation of traditional Chinese medicine,give full play to the characteristics of traditional Chinese medicine;6.The patient clear the target in their treatment and care,active participation,optimize the nursing process,improve the nursing job satisfaction.
