OBJECTIVE To explore the factors affecting the recurrence and metastasis of colorectal cancer,and to provide the basis for postoperative follow-up monitoring and prevention and treatment of recurrence and metastasis.METHODS Collected in January 2009-December 2013 in our hospital for colorectal cancer radical surgery in 578 cases of medical records.Including sex,age,surgical approach,primary tumor location,preoperative CEA,general type,histological type,degree of tumor differentiation,tumor thrombus and nerve invasion,cancer nodule,T stage,lymph node metastasis,postoperative adjuvant therapy,And the factors influencing the recurrence and metastasis of colorectal cancer were analyzed by Chi-square test and binary logistic method.RESULTS According to the inclusion criteria and exclusion criteria,450 patients with radical resection of colorectal cancer were treated with 95 patients(21.1%)with recurrence and metastasis during follow-up.95 cases of recurrence and metastasis cases,local recurrence in 30 cases,distant metastasis 79 cases.The median time to recurrence in 95 patients was 16 months.The median time to local recurrence was 16.7 months;the median time to distant metastasis was 16.0 months,and the median time to liver metastasis and lung metastasis was 12.4 months and 20 months,respectively.The total recurrence and metastasis rates of this group were 1 year,2 years and 3 years,respectively: 8.0%,14.9% and 18.2% respectively.The relapse rates were 2.4%,14.8% and 32.7%,respectively,according to TNM stage,stage I,stage ? and stage ?.Among the 450 patients,there were 79 patients with recurrent adjuvant therapy and 16 had no recurrence in patients who did not receive adjuvant therapy.The group of 259 cases of colon cancer cases,45 cases of patients with recurrence after surgery,191 cases of rectal cancer cases in 50 cases of patients with recurrence and metastasis after surgery Univariate analysis showed that age,primary tumor location,tumor differentiation,T stage,lymph node metastasis and postoperative adjuvant therapy were related factors of postoperative recurrence and metastasis.Multivariate analysis suggested that lymph node metastasis was an independent risk of recurrence and metastasis factor.CONCLUSION 1.In this group of patients,most of the recurrence and metastasis occurred within 2 years after surgery.The recurrence of colorectal cancer after radical resection is mainly distant metastasis,in which liver metastasis and lung metastasis are the most common distant metastasis site,liver metastasis earlier than lung metastasis.2.Patients with recurrent risk factors should pay attention to regular review after surgery,so that early recurrence of early treatment of cancer.
