Background and objective: Sepsis is a life-threatening organ dysfunction caused by uncontrolled response to infection.Sepsis,infectious complications and multiple organ failure(MOF)is a serious disease in patients with severe trauma,the incidence is high,the mortality rate remains high,and the pathogenesis is not clear.Genetic variation,especially single nucleotide polymorphism(SNPs),is considered to be an important determinant of individual differences in inflammatory response and clinical outcome in septic patients.This research intends to study the sepsis patients in Guangxi area with the health of advanced glycation end products receptor(RAGE)gene polymorphism distribution,analysis and development of sepsis may be related to SNP,at the same time in the analysis of the pathogenesis of sepsis and its role.Methods: According to the " Surviving Sepsis Campaign: international guidelines for management of severe sepsis and septic shock,2012",152 cases of sepsis inGuangxi(sepsis group)and 189 cases of healthy subjects in Guangxi(control group)were selected as subjects as the research object.The Matrix-Assisted Laser Desorption-Ionization Time of Flight Mass Spectrometry(MALDI-TOF-MS)were used to analyze the SNPs of RAGE :rs2070600C/T,rs184003C/A,rs2071288C/T,rs1035798G/A,rs1800624A/T and rs17846804G/A alleles and genotypes.Serum endothelin-1(ET-1)Level were detected by enzyme-linked immunosorbent assay(ELISA).The polymorphism of RAGE gene polymorphism in Guangxi population was compared with that in other regions.Compared with the plasma ET-1 level,the distribution of RAGE gene locus in sepsis group and control group was compared,to explore the relationship between polymorphism of RAGE and sepsis.Results: The distribution of RAGE gene polymorphism in Guangxi population is different from that of Chinese Pekingese,Japanese,Europeans and Africans.There was no significant difference in genotype frequency and allele frequency of SNP locus rs2070600(C/T),rs184003(C/A)and rs2071288(C/T)between the sepsis group and the control group.The level of serum ET-1 in the sepsis group was significantly higher than that in the control group {[median 7.69pg/ml,quartile spacing(3.40-24.12)pg/ml] vs [median 3.69 pg / ml,Pitch(2.41-5.38)pg/ml] P <0.05}.The mean plasma ET-1 concentration in the sepsis group was [median 24.00pg/ml,quartile spacing(23.35-38.09)pg/ml],and the mean concentration of ET-1 plasma in severe sepsis group was [median 63.60 pg / ml,quartile spacing(29.38-76.38)pg/ml],sepsis shock group plasma ET-1 average concentration of [median 34.75pg/ml,quartile spacing(13.64-71.82)pg/ml].There was a significant difference between ET-1 plasma concentration in sepsis group,severe sepsis group and septic shock group(P <0.05).Rs2070600(C/T),rs184003(C/A),rs2071288(C/T)locus,the average ET-1 level of sepsis was different.Conclusion: There were regional differences in RAGE gene polymorphism.The polymorphism of RAGE gene in Guangxi population may not be related to the susceptibility of sepsis,but the single nucleotide mutation of RAGE gene is related to the expression of ET-1 in sepsis.
