Analysis The Relevant Risk Factors Of Portal Vein System Thrombosis After Splenectomy

Objective:A retrospective analysis of the relevant risk factors with the Portal Vein System Thrombopoiesis(PVST)after Splenectomy in our hospital.Methods : Retrospective analysis from June 2013 to December 2016,the clinical data of 60 patients with splenectomy.Selecting patients' gender,age,pathological diagnosis,preoperative alanine amino-transferase(AL T),preoperative aspartate amino-transferase(AST),preoperative albumin(AL B),preoperative total bilirubin(TBIL),preoperative triglyceride(TG),preoperativedirect bilirubin(DBIL),preoperative total cholesterol(TC),preoperative platelet count(PLT),preoperative mean platelet volume(MPV),preoperative p-lateletcrit(PCT),preoperative platelet distribution width(PDW),preoperative platelet-large cell ratio(P-LCR),preoperative hemoglobin(HGB),preoperative white blood cells(WBC),preoperative prothrombin time(PT),preoperative prothrombin time activity(PTA),preoperative international standardization ratio(INR),ratio of preoperative prothrombin time(PTR),preoperative thrombin time(TT),preoperative activated partial thrombin time(APTT),preoperative f-ibrinogen(Fib),ALT after operation,AST after operation,TBIL after operation,DBIL after operation,ALB after operation,TG after operation,TC after o-peration,PLT after operation,MPV after operation,PCT after operation,PD W after operation,P-LCR after operation,HGB after operation,WBC after o-peration,PT after operation,PTA after operation,INR after operation,PTR a-fter operation,TT after operation,APTT after operation,Fib after operation,whether preoperative combined high blood pressure(HBP)and diabetes mellitus(DM),whether preoperative combined liver cirrhosis,operation method,differences of operation time and intraoperative blood transfusion as observation index.In 9 cases of patients with postoperative portal venous s-ystem thrombosis risk factors were retrospectively analyzed.Results:Finding that postoperative portal venous system thrombosis is connected with postoperative pathological diagnosis(P=0.010),whether preoperative combined liver cirrhosis(P=0.049),surgery methods(P=0.011),preoperative TBIL(P=0.020),preoperative the WBC(P=0.026)and postoperative TG(P=0.007) P<0.05,there is statistically significant;And patients' gender,age,preoperative ALT,preoperative AST,preoperative TBIL,preoperative DBIL,preoperativ e ALB,preoperative TG,preoperative TC,preoperative PLT,preoperative MP V,preoperative PCT,preoperative PDW,preoperative P-LCR,preoperative HG B,preoperative WBC,preoperative PT,preoperative PTA,preoperative INR,pr eo-perative PTR,preoperative TT,preoperative APTT,preoperative Fib and ALT after operation,AST after operation,TBIL after operation,DBIL after oper-ation,TG after operation,TC after operation,PLT after operation,MPV aft-er operation,PCT after operation,PDW after operation,P-LCR after ope rati-on,postoperative HGB,postoperative WBC,postoperative PT,postoperati ve PTA,postoperative INR,postoperative PTR,postoperative TT,postoperative APTT,postoperative Fib,preoperative combined postoperative HBP/DM,wi th and without postoperative anticoagulation promoting blood circulation t o r-emove bloodstasis and whether intraoperative blood transfusion P>0.05,t-here was no statistically significant difference;Preoperative DBIL(P=0.069)and the operation time(P=0.054)P close to 0.05,all blood indexes s uch as preoperative differential postoperative,the preoperative postoperative ALT,AST,DBIL,HGB,PLT,INR,APTT et al,for t test or rank and inspectio n,found that preoperative postoperative TBIL difference P=0.042,P<0.05,t he difference is statistically significant,combinating with related literature at h-ome and abroad,TBIL and operation time may be associated with p ost-operative PVT formati-on;preoperative ALT difference after operation,p reo-perative AST difference after operation,preoperative DBIL difference a fter operation,preoperative TG difference after operation,preoperative TC dif-ference after operation,preoperative PLT difference after operation,pre ope-rative HGB difference after operation,preoperative WBC difference aft er o-peration,preoperative MPV difference after operation,preoperative PCT di-fference after operation,preoperative PDW difference after operation,p reo-perative P-LCR difference after operation,preoperative INR difference after operation,preoperative PTR difference after operation,preoperative A PTT difference after operation,preoperative PTA difference after operation,pre-operative Fib difference after operation,preoperative TT difference afte r o-peration,preoperative PTA difference after operation,(P>0.05).Conclusions:t-he postoperative pathological diagnosis after splenectomy,operation m eth-ods,whether preoperative combined liver cirrhosis are risk factors for po-rtal venous system thrombosis after splenectomy,operating time may be a risk factor for portal system thrombosis after splenectomy,postoperati ve TBIL elevated factors may be the protection of the portal venous syst em thrombosis.
