Objective: Objective to study the characteristics of platelet rich plasma(PRP)in the treatment of limb skin defect,and to find out the most suitable treatment for elderly patients with cardiopulmonary diseases associated with no surgical indications.Methods: The patients was group by the area size of limb skin defect,the skin defect with the diameter of 8cm2 were regard as group A,and the diameter of 12cm2 was deemed as group B in the study group there was 15 patients in each group.16 patients was injured by the accident,and 14 patients was injured by the high.Likely,there were the same patients in the control group,include group C and group D.Unlikely,18 patients was injured by the accident,12 patients was injured by the high falling.the patients with diabetes,blood system diseases,and infection should be ruled out.The statistical significance wasn't detected in baseline information among the four groups(P > 0.05).A group and B group were treated with platelet rich plasma.The PRP was obtained from the elbow peripheral venous blood,and the measures were: spinning the tubes at 3500rpm/min to centrifuge the blood samples(50ml)once taken,then removing the bottom red blood cells,and spinning the tubes at 3700rpm/min to centrifuge the blood samples to 10 ml once more to prepare PRP.The prepared PRP was put in defect of skin,placed absorbable hemostatic gauze on the surface,removed residual hemostatic gauze and dried blood after 7 days,and gradually removed the dried blood in 14 days,observed the healing of the wound.The wound was cleaned and disinfected by the conventional way in the control group,then covered skin defect with recombinant human epidermal growth factor gel,observed the skin defect area changes in 7 days and 14 days.We recorded the area change of skin defect the wound in the process of treatment,the data of the routine blood total number included white blood cells,blood sedimentation,inspection,the document of the wound secretion in 7 days,14 days was analysed between groups.Result:(1)We observed that the scabby appeared in the skin defect,and there was no leakage in the wound in seventh day in group A ?group B?group C?group D.the scab skin was ruddy,the area was reduced about 1/2 in group A,1/3 in group B in 7 days in study group.The area was reduced about 1/5 in group C,and about 1/6 in group D in the control group after removed the center part of scab shell.Comparing to the control group,The area of group A and B was significantly reduced(P < 0.05).(2)the wound healed well and the growth epidermal was ruddy after removing the residual scab shell in group A;Group B showed significantly reduced wound healing of skin defect,about 3.0-3.6cm2 was not healed,the granulation tissues was ruddy in surrounding skin;In group C,the wound was reduced by about 2.0cm2,the wound was not healed,and the wound surface was a little exudate;The wound of D group was reduced by about 2.4cm2,and the wound was not healed,the granulation tissue was ruddy in surrounding growth skin.The area of group A and group B was significantly reduced than the control groups(P < 0.05).(3)The baseline information(age,sex,skin defect parts)was no statistical difference(P > 0.05)among group A,B,C,D.Conclusion:(1)The therapy of PRP has a good advantage in limb skin defect with the diameter of 8cm2,12cm2 than the treatment of a recombinant human epidermal growth factor gel.(2)the therapy of PRP can promote wound healing,Shorten hospitalization time,reduce laid-in-bed complications,reduce the economic burden in the treatment of the skin defect in the older patients with espiratory system disease.
