Objective: Surgical resection of butterfly-like corpus callosum gliomas(tumor invasion of the corpus callosum and bilateral cerebral hemispheres)has both bilateral and unilateral approaches.By comparing these two approaches,Clinical significance and value of corpus callosum glioma.Methods: Forty-two patients with glaucoma-like glioma diagnosed during the period from March 2013 to December 2016 were enrolled in this study.(The preoperative MRI examination was performed on the bilateral corpus callosum and the hemispheres on both sides of the brain Bit,the shape of butterfly-like,surgical resection,and postoperative pathological diagnosis of glioma).There were 29 cases of bilateral frontal lobe corpus callosum glioma,5 cases of bilateral parietal corpus callosum glioma,4 cases of bilateral temporal corpus callosum glioma,4 cases of bilateral lateral corpus callosum glioma.23 males and 19 females,the youngest 14 years old,the largest 75 years,mean age 44.6 years old.Among them,20 cases were treated with bilateral approach,22 cases were treated by unilateral approach,and postoperative CT and MRI were reviewed for 24 hours and 72 hours.Through the retrospective analysis of the bilateral approach and unilateral approach to the operation time,intraoperative blood loss,the degree of resection of the tumor,the incidence of postoperative complications,the average length of hospital stay and other indicators,with statistical methods,The clinical value of lateral approach.Results: The average operative time was 3.8 ± 0.3 hours in 22 patients with unilateral approach,the average intraoperative blood loss was 386 ± 55 ml,the total tumor was 11 cases,the postoperative hemiplegia was 1 case,the average length of hospital stay was 14.3 ± 1.1 days.The mean operative time was 5.3 ± 0.5 hours in 20 patients undergoing bilateral incision,the average intraoperative blood loss was 534 ± 98 ml,the total tumor was 11 cases,and the postoperative hemiplegia was 6 cases.The average length of hospital stay was 24.2 ± 3.2 days.There was no postoperative death in both groups.Conclusion: The operation time,intraoperative blood loss,the incidence of postoperative hemiplegia and the average number of hospitalizations were lower than those of bilateral approach,the difference was statistically significant.and the degree of tumor resection was not statistically significant.The Unilateral approach excision of the butterfly-like corpus callosum glioma is safer,has a better therapeutic effect and clinical value.
