ApoB/ApoA-? Ratio And The Severity Of Coronary Artery Disease In The Population Of XINJIANG Province

Objective:For decades,LDL has been widely recognized as the major atherogenic lipoprotein and the primary target for prevention of coronary heart disease(CHD);while,growing evidences has showed that other triglyceride-rich lipoproteins,such as very low-density lipoprotein(VLDL)and intermediate density lipoprotein(IDL)carry atherogenic potential as well.As each one of LDL,IDL and VLDL particles carries only one apolipoprotein B-100(Apo B-100)molecule,the total Apo B value represents the total number of potentially atherogenic lipoproteins,Recent studies suggest that apolipoproteins Apo A1 and Apo B may improve CHD risk assessment by identifying more high-risk individuals than the usual lipid profile alone.However,the targets for the optimal treatment of CHD remain unknowen.Further studies are necessary to determine whether Apo B and Apo A1 are superior to conventional lipid parameters as predictors of CHD.Methods: We consisted of 2017 consecutive patients admitted to xinjiang medical university affiliated first hospital in March,2012-April,2015 who has been diagnosed CHD by coronary angiography.The serum was assayed on admission for HDL,LDL,Apo AI and Apo B,and the other lipid parameters.Demographic and biochemical characteristics of CHD patients were compared according to the tertiles of Gensini scores(GS,low group < 12 points n = 642;intermediate group 12–48 points,n = 708;high group > 48 points,n =668).Association between the ratios of apolipoprotein(apo)B and apo A-1(apo B/apo A1),total cholesterol and high density lipoprotein cholesterol(TC/HDL-C),triglycerides and HDL-C(TG/HDL-C),low density lipoprotein cholesterol and HDL-C(LDL-C/HDL-C)and GS were evaluated using the receivers operating characteristic(ROC)curves and multivariate logistic regression models.Results: We found that apo B/apo A1 ratios were significantly higher in higher gensini score group.The ratio of apo B/apo A-1,TC/HDL-C,LDL-C/HDL-C were correlated with Gensini scores.Area under the ROC curves for predicting high Gensini scores in the ratios of apo B/apo A-1,TC/HDL-C,LDL-C/HDL-C and TG/HDL-C were 0.571,0.563,0.544 and 0.558,respectively(P < 0.005 for all).Conclusion:Compared with other lipid parameters,the ratio of apo B/apo A-1 appears to be more significantly correlated with the extent of coronary artery lesions in CHD patients of XINJIANG province,is an independent predictor for the severity of CHD in those patients(OR = 1.697,95%CI: 1.129-2.551,P = 0.011).
