ObjectiveTo study the risk factors for diabetic foot happen,of this study was to evaluate the correlation of blood glucose fluctuations with diabetic foot and the diabetic foot amputation.MethodsA total of 91 cases of patients diagnosed with diabetic foot were divided into 5groups according to Wagner classification,these all patients were in hospital of Bethune first hospital of Jilin university between January 2014 and December 2016.123 patients with type 2 diabetes mellitus without diabetic foot in hospital were selected in the same period in the endocrine metabolism.Diabetic foot patients according to Wagner grade 1 ~ 5 will be divided into 5 groups,one group of 23 people,2 groups of 12,3 groups 24 people,four groups of 22 people,5 groups of 10 people.Eliminate acute complications of diabetes,the recent application of diuretics,combination of malignant tumor,and severe heart,liver,renal insufficiency,patients with incomplete information.Record all of the patients age,course of the disease,body mass index,the second day admission venous blood was collected on an empty stomach blood total cholesterol,high density lipoprotein cholesterol,low density lipoprotein cholesterol,triglyceride,serum total protein,serum albumin,urea nitrogen,creatinine,uric acid,glycated hemoglobin values.After admission first complete fasting,2 hours after breakfast before lunch,after lunch 2 hours,2 hours before dinner,after dinner,just before bed,finger tip blood sugar,the hospitalization days more,take first complete seven times after admission blood glucose,fasting blood glucose levels and continuous recording 5 days.We chose three indexes to evaluate blood sugar fluctuation,respectively standard deviation of blood glucose(SDBG),fasting plasma glucose-coefficient variation(CV-FPG),the largest amplitude of glycemic excursions(LAGE).Statistical methods: input data using Excel spreadsheet,and establish the database after checking.Using SPSS software,the approximate normal distribution measurement data of the application(`x±s),counting data by chi-square test,the differences between the measurement data set by variance analysis,risk factor analysis using logistic regression analysis,inspection level of alpha = 0.05.Results1.The influence factors of diabetic foot with gender,age,blood albumin,TG,urine A/C.Men's risk for diabetic foot is 3.239 times that of women;Compared with patients younger than 50 years old,patients over 50 years old respectively was2.521 times the risk for diabetic foot.Blood albumin over 40 g/l risk for diabetic foot is0.171 times of patients whose blood albumin lower than 40g/l.Compared with TG <1.8mmol//l,people whose TG over 1.8mmol/l have lower risk for diabetic foot;Urine A/C 30mg/g or higher risk for diabetic foot is 4.702 times of the urine A/C < 30mg/g.2.Compared with NDF group,DF amputation group of hemoglobin Alc,fasting blood glucose,blood sugar 2 hours after breakfast,blood sugar before and 2 hours after lunch,blood sugar before and 2 hours after dinner,Blood sugar before sleeping,5 day average fasting blood glucose,LAGE were higher(P < 0.05).DF the amputation group of fasting glucose levels higher than the NDF,but lower than that of DF amputation group(P < 0.05).3.Amputation group and simple expansion debridement fasting blood glucose,blood sugar,five days before the dinner average fasting glucose were higher than those who did not have an operation(P < 0.05);Amputation group blood sugar 2 hours after lunch,LAGE were higher than those who did not have an operation,and the difference was statistically significant(P < 0.05).Conclusion1.The influence factors of diabetic foot include gender,age,blood albumin,TG,urine A/C.male,age > 50,blood albumin < 40 g/l,TG < 1.8mmol/l,urine A/C > 30mg/g tendency for result in an increased risk of diabetic foot.2.High blood glucose,the blood glucose fluctuations in one day LAGE are risk factors for diabetic foot happen and patients with diabetic foot under amputation.
