Objectives:To analyze the relationship between chronic insomnia with the cardiac diastolic dysfunction in patients with stable angina and its possible mechanism;to summary the distribution of patients with TCM syndrome type,and to compare the difference between the diastolic function between different syndrome types.In order to provide help for clinical diagnosis and treatment of cardiac diastolic dysfunction.Methods:183 patients were randomly selected who was with stable angina,and conformed to the standard of diagnosis of chronic insomnia.According to the course of insomnia,those patients were divided into four groups the first group of 6 months to 2 years,the second group of 2 years to 5 years,the third group of 5 to 10 years,the fourth group>10 years.Compare the general condition of 4 groups of patients,and observe hs-CRP,heart diastolic function level,SDNN of HRV(the heart rate variability),and the night average heart rate;Analyze the relationship between course of insomnia with the E/E’ rate、SDNN、the average heart rate at night,and the hs-CRP;Based on analysis of TCM syndrome differentiation,analyze the relationship between diastolic function and chronic insomnia with TCM syndrome typeResults:1,Pearson correlation analysis showed that the course of insomnia positively related with E/E ’(r = 0.712,P<0.01),and negatively related with SDNN(r =-0.516,P<0.01),positively related with night average heart rate(r = 0.742,P<0.01),and positively related with the hs-CRP(r = 0.715,P<0.01).2,TO control variables of age,gender,hypertension,diabetes,hyperlipidemia,partial-correlation analysis showed that the course of insomnia positively related with E/E’(r = 0.710,P<0.01),and negatively related with SDNN(r =-0.509,P<0.01),positively related with night average heart rate(r = 0.739,P<0.01),and positively related with the hs-CRP(r = 0.717,P<0.01).3,Spearman analysis showed that diastolic function level were positively related with insomnia level(r = 0.659,P<0.05).4,The patients’ TCM syndrome type distribution ratio were different,from high to low,followed by phlegm obstruction heart pulse syndrome,qi and Yin deficiency syndrome,blood stasis syndrome,qi deficiency with blood stasis syndrome,qi deficiency with blood deficiency syndrome,syndrome of Yin is cold,and Yang deficiency syndrome,heart and kidney Yin deficiency syndrome.Comparison between groups of phlegm obstruction heart pulse syndrome,deficiency of qi and blood stasis syndrome with blood deficiency syndrome,qi and Yin deficiency syndrome exists statistical difference(P<0.05),Comparison between groups of blood stasis syndrome with Yin is cold syndrome,and Yang deficiency syndrome exists statistical difference(P<0.05),deficiency of blood deficiency syndrome and qi stagnation and blood stasis exists statistical difference(P<0.05),no statistical difference between other syndrome types.Conclusion:1,In patients with stable angina,chronic insomnia has relationship with cardiac diastolic dysfunction,the longer course insomnia,the lower diastolic function;2,Along with the increasing course of insomnia,patient’s SDNN is prone to be lower,night average heart rate and hr-CRP are prone to be higher,that may be the mechanism why chronic insomnia realted cardiac diastolic dysfunction related in patients with stable angina;3,In patients with stable angina,the occurrence of chronic insomnia with different TCM syndrome distribution,the occurring rate of phlegm obstruction heart pulse syndrome arteries might be higher.
