Objective:To compare the early efficacy of repeted hepatectomy,microwave ablation and transcatheter arterial chemoembolization(TACE)in the treatment of recurrent hepatocellular carcinoma within Milan criteria.Methods:A retrospective analysis of 47 patients within Milan criteria(single tumor diameter ≤ 5cm or multiple tumor ≤ 3 and maximal diameter ≤ 3 cm)of liver cancer after radical surgery recurrent patients with clinical data from January 2009 to January 2015 in China-Japan friendship hospital of Jilin University.12 patients underwent repeted hepatectomy,20 patients underwent microwave ablation,and 15 patients underwent transcatheter arterial chemoembolization.The incidence of duration of hospitalization,complication and early survival were compared between the resection group,the microwave ablation group and the TACE group.Results:The incidence of treatment-related complications rate was 50.0%(6/12)in the resection group,which was significantly higher than that in the microwave group 10.0%(2/20)and the TACE group 13.3%(2/15)(p=0.018);The duration of hospitalization of resection group was(16.41 ± 4.96)d,which was obviously more than that in the microwave group(8.70 ± 2.96)d and the TACE group(7.40 ± 1.84)d(p=0.000);The tumor-free survival rate of the resection group was significantly higher than that of the microwave ablation group and the 1-,2-,3-year were 75.0%、38.9%、19.4%,and 50.0%、11.1% 、0%(p=0.044);The survival rate of the resection group and the microwave ablation group was significantly better than that of the TACE group,The 1-,2-,3-year were 83.3%、58.3%、33.3%,and 80.0% 、45.0%、29.2%,and 41.7% 、14.3%、7.1%(p=0.010).Conclusion:1.Reoperation provides better tumor-free survival rate than Microwave ablation,in accordance with the Milan criteria of recurrent hepatocellular carcinoma resection.2.Microwave ablation can be achieved with reoperation of the early survival rate,and the effect is better than TACE.3.Microwave ablation and TACE compared with reoperation with minimally invasive,rapid postoperative recovery,shorter hospital stay and so on.
