Objective:Through the determination of serum cystatin c and retinolbinding protein 4 level, and analyze its correlation with the occurrence and development of chronic heart failure, and explore the effects of chronic heart failure in the process and prognosis. Methods:In this study,we selected 60 cases of patients with chronic heart failure, from September 2015 to September 2016 in Department of Cardiology,China Japan Union Hospital of Jilin University, there weremale 35 cases, female 25 cases;at the same time and selected non heart failure patients 30 cases as the control groupand,19 cases were male,11 cases were female.Between ages 25-80 years old,the average age was 63(49,73)years old.According to the basic causes of heart failure were divided into 10 cases of hypertension, coronary heart disease in 30 cases, heart valve disease in 8 cases, cardiomyopathy in 10 cases, congenital heart disease in 2 cases.According to the New York Heart Association(NYHA)heart function classification standard, evaluation of cardiac function, and cardiac function is divided into II ~ IV,and the patients were divided into 4 groups:(1)the control group(non heart failure patients group)was 30 cases.(2)cardiac function grade II group was 18 cases.(3)cardiac function III group was 20 cases.(4)patients with cardiac function IV grade group was 22 cases.Using SPSS 19 statistical software processing, experimental measurement data using the mean and standard deviation((?)+ s),median, four percentile interval representation;count data using rate and constituent ratio representation.Between group and group compared with the t test and analysis of variance normal distribution, two two comparisons using multiple comparison(LSD)method,If it does not conform to the normal distribution or the variance is not homogeneous, using nonparametric test, and using linear correlation analysis to analyzethe relationship between cystatin c,retinolbinding protein 4,N-terminal pro-brain natriuretic peptide(NT-pro BNP)and heart function in patients with heart failure, the difference was statistically significant with P < 0.05.Exclusion criteria: acute pulmonary embolism;acute and chronic liver disease;severe infectious disease;autoimmune diseases;immunodeficiency disease;malignant tumor and blood disease;pregnancy and psychiatric patients.Reasults:1?The levels of serum cystatin C and retinol binding protein 4 increases with the increase of N-terminal pro brain natriuretic peptide levels,by correlation analysis, serum cystatin C and retinol binding protein 4 levels were positively correlated with the levels of N-terminal pro brain natriuretic peptide.2?The levels of serum cystatin C and retinol binding protein 4 increased with the increase of left ventricular end diastolic diameter,by correlation analysis, serum cystatin C and retinol binding protein 4 levels were positively correlated with left ventricular end diastolic diameter.3?The levels of serum cystatin C and retinol binding protein 4 were increased with the decrease of ejection fraction,by correlation analysis, serum cystatin C and retinol binding protein 4 levels were negatively correlated with ejection fraction. Conclusion:Through the determination of serum cystatin c and retinol binding protein 4 level,it is proved the relationship between the two and ventricular remodeling in patients with heart failure,it is related to the occurrence and development mechanism of chronic heart failure, and can be used as an important index to evaluate the progress and prognosis of chronic heart failure.
