Objective:To compare the diagnostic value of computer tomography, magnetic resonance imaging, contrast enhanced ultrasonography for pancreatic cystic lesions.Method:Detabase including Embase,Pubmed,Medline,CBM,Wanfang datebase,CNKI are searched for the literatures related to the methods mentioned above for diagnosing pancreatic cystic lesions. Two different systematic reviewers independently screen the articles according to the inclusion and exclusion criteria and then assess the quality of included literatures.specificity, positive likelihood ratio,Sensitivity, negative likelihood ratio, diagnostic odds ratio,Q index, area under curve are pooled and summary receive operating characteristic curve is drawn by Meta-disc software.the diagnostic value of the above imaging diagnostic methods for pancreatic cystic lesions is compared by the Q value's Z-examination.Result:Finally there were16 articles being analysed, which include 14 English articles and 2 Chinese articles. The results of CT are that the pooled sensitivity is 0.74[95%CI(0.67-0.80)],the pooled positivelikelihood ratio is 3.66[95%CI(1.78-7.51)],the pooled specificity is0.79[95%CI(0.74-0.84)],the pooled negative likelihood ratio is0.30[95%CI(0.22-0.39)],the pooled diagnostic odds ratio is15.96[95%CI(5.14-49.57)],the pooled area under curve is 0.8753.The results of MRI are that the pooled sensitivity is 0.73[95%CI(0.65-0.80)],the pooled specificity is 0.87[95%CI(0.81-0.92)],the pooled positive likelihood ratio is 6.33[95%CI(4.00-10.03)],the pooled negative likelihood ratio is 0.30[95%CI(0.22-0.39)],the pooled diagnostic odds ratio is 32.60[95%CI(15.04-70.68)],the pooled area under curve is0.9257.The accuracy of pairwise comparisons to different diagnostic imaging methods all show P>0.05,which suggests that the difference is not statistically significant. Conclusion:The results of the Meta-analysis indicate that MRI has higher specific degrees in the diagnosis of pancreatic cystic lesions, while pairwise comparisons show that there is no statistically significant difference among CT,MRI and ontrast enhanced ultrasonography in the accuracy of diagnosis.
