OBJECTIVE: Pancreatic fistula is a serious complication with high morbidity rate,which often happens after pancreaticoduodenectomy and significantly hinders the rehabilitation of patients.This study explores the role of early monitoring indicators after surgery on providing relatable forecasts of pancreatic fistula through analyzing the related indexes statistics for the patients who receive pancreaticoduodenectomy in our hospital.This study is vital to make early prediction of postoperative complications and take relevant preventive measures.METHODS: The clinical data of 116 patients with pancreaticoduodenectomy in Second Hospital of Jilin University from January 2013 to December 2016 was retrospectively analyzed.According to the definition of pancreatic fistula proposed by international study group on pancreatic fistula,we make a single factor analysis for the trend of change in blood amylase,drainage fluid amylase,C-reactive protein,leukocyte,albumin and related indexes in the first three days after surgery.Furthermore,a multi-factor analysis was made in which pancreatic fistula andnon-pancreatic fistula are dependent variables,and the relative predictive index of Pancreatic fistula are independent variables,by running dual Logistic regression analysis.RESULT: 116 patients receive pancreaticoduodenectomy.16 patients of them suffer from pancreatic fistula,and two patients died.The incidence of pancreatic leakage was 13.8%,and death rate was 12.5%.Logistic regression analysis showed that intraperitioneal drainage amylase on the first day after the operation,blood amylase and serum albumin in the third day after operation were the independent risk factors for pancreatic fistula.CONCLUSION: The level of peritoneal drainage fluid amylase and blood amylase on the first day after pancreaticoduodenectomy and the serum albumin level on the third day after pancreaticoduodenectomy have a significant role in providing reliable forecasts of pancreatic fistula.
