ObjectivesThe current study was conducted to evaluate the effict of whey protein combined with vitamin D and E to on muscle mass,strength and function of the old people with sarcopenia.MethodsDuring October 1,2015 to January 31,2016,a randomized,controlled,double-blind,trial was conducted.The subjects were 60 sarcopenic elderly adults were included in the current study.The random numbers were generated by the computer,matched by sex and age(30 subjects in intervention group,and another 30 subjects in placebo group).The subjects in interventtion group received nutritional supplement rich in vitamin D,E and whey protein,and they consume twice daily(22g every time)for 6 months.The placebo group received an equal energy and equal weight control product to consume twice daily for 6 months.Besides,the subjects were asked to maintain their usual habits and followed up every month.Primary outcomes of handgrip strength and RMSI,and secondary outcomes of chair-stand test,6-m gait speed,timed-up-and-go test,appendicular muscle mass(by bioimpedance analysis,BIA),nutritional status,and quality of life were measured at baseline and month 6 of the intervention.Baseline data were presented by means and standard deviation.An ANCOVA with sex as a covariate was used to evaluate the effect of nutritional supplementation on RMSI and handgrip strength.The analysis of independent samples t tests were used to evaluate the effect of nutritional supplementation on chair-stand test,6-m gait speed,appendicular muscle mass(by bioimpedance analysis,BIA),and quality of life.ResultsCompared with the placebo group,after 6 months of nutritional supplementation,the skeletal muscle of the subjects in the intervention group increased by 0.23±1.07 kg,while that of the subjects in the placebo group was reduced by 0.25 ±0.71 kg,and the difference was statistically significant(P=0.043).The changes of handgrip strength of the subjects in the intervention group were 1.9±4.24 kg,those of the subjects in the placebo group were-0.88±3.20 kg,and the difference was statistically significant(P=0.006).The change of RSMI of the subjects in the intervention group were 0.08±0.38 kg/m2,while that of the subjects the placebo group were-0.13±0.31 kg/m2,and the difference was statistically significant(P=0.021).Their changes of time to complete 6-m walk in the intervention group were-1.01±1.50 s,those of the subjects in the placebo group were-0.02±5.17 s,and there was no statistically significant difference(P=0.312).After 6 months of intervention,the serum triglyceride of the participants in the intervention decreased by 0.23±0.52 mmol/L,while that of the participants in the placebo group increased by 0.01±0.41 mmol/L,and the difference was statistically significant(P=0.046).The serum insulin like growth factor-1 in intervention group of subjects in the intervention group increased by 14.18±28.33ng/m L,those of the subjects in the placebo group decreased by 1.13±17.96 ng/m L,and the difference was statistically significant(P=0.015).The serum interleukin-2 of the participants in the intervention group increased by 40.17±487.18 pg/m L,while that of the subjects in the placebo group increased by 633.3±1362.27 pg/m L,and the difference was statistically significant(P=0.029).In the intervention group,the score of SF-36 role-physical of the participants increased by 17.39±16.92,while that of the subjects in the placebo group was reduced by 2.28±17.01 g,and the difference was statistically significant(P<0.001).The score of SF-36 role-emotional of the participants in the intervention group increased by 12.98±13.70,while that of the subjects in the placebo group was increased by 2.20±14.23 g,and the difference was statistically significant(P=0.004).ConclusionsCombined supplementation with whey protein,vitamin D and E can improve muscle mass and strength,the quality of life,the status of serum triglyceride,insulin like growth factor-1,and interleukin-2 in patients with sarcopenia.
