Clinical Analysis Of 61 Cases Of Peri-brain Stem Tumor

Objective To analyze the clinical manifestations,imaging features,surgical methods,postoperative complications and prognosis of peri-brain stem tumors.Methods We retrospectively analyzed the clinical data of 61 patients with peritumoral brain tumor from March 2013 to May 2016 in our hospital.The symptoms and signs,imaging findings,operation methods,postoperative complications disease and prognosis to be summarized analysis.Outcomes Brainstem function is very important,peripheral brain cancer clinical features of complex and diverse,headache,pyramidal tract signs and brain function damage and other symptoms than the preoperative remission;imaging examination can determine the extent of tumor compression from different angles,The choice of surgical approach.There were 42 cases(68.8%)of subtotal resection and 12 cases(19.7%)of subtotal resection,7 cases(11.5%)were removed and 3 cases died(4.9%).Pathological examination results: 13 cases of schwannoma,medulloblastoma in 8cases,8 cases of ependymoma,meningioma in 7 cases,5 cases of hemangioblastoma,epidermoid cyst in 4 cases,4 cases of astrocytoma,3 cases of pharyngeal tumor,3cases of macrophage hemangioma,2 cases of giant pituitary tumor,2 cases of glioblastoma,1 case of germ cell tumor and 1 case of metastatic carcinoma.The main complications were: hyperthermia 10 cases,epilepsy 4 case,1 case of coma,1 case ofcentral dyspnea,15 cases of dysphagia,19 cases of limb weakness,4 cases of facial paralysis,4 cases of hearing loss,diplopia in 7 cases,13 cases of pulmonary infection,intracranial infection in 6 cases 5 cases of cerebrovascular disease,5 cases of cerebellar silent morbidity,5 cases of subdural effusion,3 cases of subcutaneous effusion,12 cases of hydrocephalus and 1 case of cerebral hemorrhage.After the rehabilitation exercise,the symptoms have improved.After 3 to 6 months follow-up,the quality of life of patients was assessed,the use of Barthel index rating scale,45 cases 60 points or more,9 cases 40-60 points,4 cases 20-40 points.60 points for the prognosis is good,60 points for the poor prognosis.Conclusion Peripheral brain cancer clinical manifestations of neurological impairment,more complications.Imaging examination of the periapical brain tumor diagnosis and treatment of important value.Surgical approach should consider the location of the tumor,size,blood supply,oppression brain stem direction,as far as possible in the case of total resection of the tumor,to avoid nerve traction and injury.Peritoneal stem cancer complications are more serious,the morbidity is high,part of the prognosis is good,improve the quality of surgery is the key to actively prevent and deal with complications is conducive to improving the prognosis of patients with follow-up found that postoperative hydrocephalus patients with prognosis difference.
