Background and ObjectiveDiabetes is a common chronic metabolic endocrine disease, which causes the carbohydrate, fat and protein metabolism disorder, cause the whole body structure and function of various organs damage. The disease in middle-aged and old,the prevalence increased with age, 2007-2008 national diabetes survey data show that the elderly aged over 60 diabetes prevalence was 20.4%, the estimate of about 35.38 million, accounting for 38.1% of the total number of cases. According to the development trend of aging population in our country, indicate the number of elderly diabetes will increase significantly. Diabetic vascular complications are the leading cause of death in diabetic patients, about 80% of the patients died of coronary heart disease (CHD),stroke and other cardiovascular complications. Diabetes vascular damage mainly for atherosclerosis, and diabetes often and hypertension,hyperlipidemia, obesity and other risk factors for atherosclerosis, further damage to the artery. So early detection, early treatment diabetic arterial damage, can greatly reduce the disability rate and mortality.Carotid intima-media thickness increase or the formation of atheromatous plaque, has become the judge effective indicators of atherosclerosis,diabetes, hypertension, coronary heart disease, stroke and other high-risk independent risk of disease of heart head blood-vessel. In recent years with color ultrasound carotid intima - middle thickness and carotid artery plaque has been widely used in clinical, with a non-invasive, easy to operate, correlation is good, price is low wait for an advantage, easy to get the recognition of the old people.In elderly patients with type 2 diabetes, this study as the research object, mainly discusses the elderly with type 2 diabetes and the relationship between atherosclerosis and its clinical significance.Sduty Object and MethodsSduty ObjectThis study from January 2015 to January 2016 in the first affiliated hospital of zhengzhou university elderly endocrinology hospital make a diagnosis and give treatment of the clinical data of 151 cases of elderly diabetes patients were retrospectively analyzed. The male 80, female 71; Aged 60?87 years, mean age(69.67-0.58).Sduty MethodsAccording to the results of carotid artery colour to exceed, according to the carotid artery atherosclerosis is divided into three groups: carotid atheromatous plaque group (CAS) 59 patients, IMT acuity 1.2 mm; Membrane thickening in the carotid artery - group of thickening (IMT) 50 cases, 1.0 mm or less IMT < 1.2 mm;Carotid artery - membrane normal group and normal group (IMT) in 42 cases as control group, the IMT < 1.0 mm.Collected at the same time the research object of gender, age, height, weight,duration of diabetes, smoking history, hypertension history, such as basic and determination of fasting blood glucose (FBG),glycosylated hemoglobin (HbAlc),triglycerides (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL -C), high density lipoprotein cholesterol (HDL - C) and 24 hours urine trace albumin and other related test are analyzed.Statistical methodsUsing SPSS 17.0 statistical software to analyze data.Using the Kolmogorov Smirnov has method to normality test, measurement data mean ±standard deviation(X ±s) said. CAS,IMT thickening,BMI,smoking history,hypertension prevalence rate duration of diabetes, HbAlc, higher than that of normal IMT group, HDL - C lower than normal IMT group, the difference was statistically significant (P < 0.05).?CAS group of BMI, smoking history, hypertension prevalence, HbAlc group is higher than the IMT thickening, thickening of HDL - C below IMT group, the difference was statistically significant (P < 0.05).?CAS and IMT thickening group as a whole, the carotid artery - membrane thickening as dependent variable, in the course of diabetes, BMI, smoking history,hypertension prevalence, HbAlc, HDL - C as the independent variable in logistic regression analysis, the results showed that diabetes duration, HDL - C two variables was statistically significant (P < 0.05),OR 1.34 and 022 respectively,tip is duration of diabetes risk factors of carotid intima-media thickness, HDL - C is the protective factors.ConclusionDiabetes duration ? HDL - C and elderly patients with type 2 diabetes there is a close independent correlation of carotid atherosclerosis, diabetes prevention, control blood sugar, elevated HDL - C level helps reduce the formation of elderly patients with type 2 diabetes atherosclerosis, for the prevention and treatment of diabetes has important clinical significance.
