Objective:To investigate the application value of carbon nanoparticle tracer in different dosages for thyroid carcinoma surgery under the concept of precision medicineMethods:A total of 60 subjects hospitalized in general surgery department of 210 hospital with papillary thyroid carcinoma(2015-12 to 2016-12)was recruited in this study.All subjects were diagnosed as highly suspected malignant pathological changes by either fine needle aspiration or color Doppler ultrasound.The dimensions and weights of the thyroids were predicted by ultrasound reports,and the patients with thyroid weight of 23gą2g were selected for further investigation.According to the different dosages of carbon nanoparticle tracer,all subjects were divided into three groups by random number method,where 0.2ml injection for group A(20 cases),0.3ml injection for group B(20 cases)and 0.4ml injection for group C(20 cases).Lymph node detections,black staining lymph nodes,revealing parathyroid glands,incidental resection and carbon nanoparticle overflow were recorded respectively during and after operation.The optimized coloration dosage of carbon nanoparticle tracer in thyroid carcinoma surgery was obtained by statistical analysis.Results:Lymph node detections numbers for group A,group B and group C were 90,150 and 158 respectively after carbon nanoparticle injection;statistic significance in average lymph node detection number was observed within these three group.(P<0.001),among which group B and group C results were significantly higher than that of group A(P<0.001).No statistic significance was found between group B and group C(P>0.05);statistic significance in black staining lymph nodes was discovered in three group(P<0.001),where group B and group C were significantly higher than group A(P<0.05).however,no significance was obtained between group B and group C(P>0.05).Parathyroid glands revealing numbers were recorded as 38,51,53 for group A,group B and group C,and statistic significance was detected(P<0.05).Group B and group C were significantly higher than group A in average parathyroid glands revealing(P<0.05),but no statistic difference was found between group B and group C(P>0.05).No statistic significance was observed in parathyroid glands incidental resection among three groups(P>0.05).Significant difference was gained within these three groups with regard to surgical field polluted by carbon nanoparticle overflow.Its rate in group C was significantly higher than that of group A and group B(P<0.05),while no significance was seen between group A and group B(P>0.05).No toxic and side effect or inflammation was reported by all 60 subjects during or after operation,indicating carbon nanoparticle injection is a lymphatic tracer with high safety.Conclusion:For thyroid weight of 23gą2g,0.3ml carbon nanoparticle injection during operation can guarantee optimal results of parathyroid negative development and central group lymph nodes coloration;probability of polluted surgical field increased with a larger dosage of carbon nanoparticle;carbon nanoparticle improved the detection rate of central group lymph nodes,resulting in a more thoroughly lymphadenectomy;better differentiation and protection of parathyroid can be achieved by applying carbon nanoparticle during thyroid carcinoma surgeries.
