New medical reform in the proposed classification and two-way referral,aimed at macro-control,coordination of large-scale medical institutions and primary health care between the medical resources to achieve the rational use of limited resources.At the same time,to encourage more patients to the first diagnosis of primary health care institutions,in order to try to change the residents of the existing treatment habits,but also to solve large medical institutions,medical resources,uneven distribution of the problem.However,for patients,the traditional concept of primary health care is limited,it is difficult to ensure the quality of medical care,a hindrance to the implementation of this policy key.In addition,the patient's focus on medical services is not limited to the level of care,more including the quality of service,waiting time(waiting for cost),medical environment.Therefore,considering the above factors,how to adjust the operation strategy of the primary hospitals and large hospitals,to achieve their own goals at the same time to promote the development of hierarchical treatment and two-way referral in the field of medical operation management needs to be resolved and has practical significance.This paper considers a two-level referral medical service system consisting of a community hospital with the aim of maximizing service capacity and a top three hospital with profit maximization,in considering the quality of medical care at the same time,adding delay-sensitive patients to seek medical treatment behavior,study the referral system pricing and capacity planning,for the top three hospitals and community hospitals to find the best balance,to help referral system in the medical unit to get their best solution,to maximize the profits of the top three hospitals and community hospital service capacity to maximize,while helping patients to better seek medical treatment.First of all,according to the patient's choice behavior,the use of M/M/1 queuing model to discuss the "patient stay time",with the help of customer perceived value and utility function to build the model,and then build the first patient to the top three hospitals to obtain the utility model and the first visit to the community hospital utility model.By comparing the utility value of the two,to determine the patient first choice of which hospital,in the theoretical basis at the same time,to provide practical reference to help patients to better choices,but also for community hospitals and the top three hospitals how to better attract patients,improve the level of medical services to provide a reference.Second,the analysis of patients in the choice of the hospital,the community hospital how to set their own service capacity to accommodate and serve more patients,while making the patient's utility value is improved,so the choice of community hospitals for the first visit.In addition,based on the design of hospital service capacity,the introduction of government subsidies,through the calculation and analysis of the final government subsidies given the threshold,if and only if the government subsidy reaches this value,the patient will choose the community hospital to go to the first diagnosis,community hospitals will not be out of market competition because of loss.Finally,the profit model of the top three hospitals is constructed,and the pricing strategy of the top three hospitals is studied.Through the construction of the top three hospital profit model to solve the different circumstances of the top three hospitals in the optimal pricing strategy,including:allow independent pricing and government guidance pricing two cases.Combined with two major influencing factors:the quality of care and the latency of patients with delayed sensitivity to discuss the impact of medical quality on patient arrival rates on medical service prices and waiting costs.In addition,based on the background of the current hierarchical diagnosis and treatment,the two-way referral system vigorously promote the background,how to optimize the operation of the top three hospitals and community hospitals to achieve win-win situation,put forward some practical suggestions and countermeasures.
