The Therapeutical Effect Of Qingyitang Combined With Magnesium Sulfate On Acute Biliary Pancreatitis

Objective:59 cases of acute biliary pancreatitis from the First Affiliated Hospital of Dalian Medical University were analyzed.The goal is to investigate the clinical efficacy of Qingyitang combined Magnesium Sulfate in the treatment of severe biliary pancreatitis,and to provide clinical experience and scientific basis for the cure rate and improvement rate of acute biliary pancreatitis.Methods:Clinical data of patients with acute abdomen surgery of the First Affiliated Hospital of Medical University in 59 cases of acute biliary pancreatitis had been retrospectively analyzed.The cases were divided into MgS04 group and Qingyitang +MgS04 group according the case report.The MgS04 group were given MgS04 in the treatment of regular medicine.However,the Qingyitang + MgS04 group were given Qingyitang combined with MgS04 in the treatment of regular medicine.The index of the two group patients were detected and recorded,these index included abdominal pain,bloating relief time,gastrointestinal decompression duration,fasting time,hospitalization time and prognosis in 24 hours,3 days,7 days;and also included blood amylase,leukocyte count,total blood bilirubin level.Local complication rate and conversion rate and other clinical parameters were analyzed between two groups.Results:1.The general index(age,gender)between two groups were no statistical difference(P>0.05).Results show that there is no difference in age and gender between the two groups of patients.2.The comparison analysis of laboratory indexes in 24 hours,3 days,7 days after admission between the two groups:serum amylase level in 24 hours after admission,third days,seventh days were significantly decreased compared with MgS04 group each time(Ftime=88.71,Ptime=0.000<0.05),and the Qingyi Decoction combined group decreased more significantly(Finteraction=6.425,Pinteraction=0.004<0.05).The white blood cell counts in 24 hours,3 days,7 days after admission showed a decreased trend in two group(Ftime=61.18,Ptime =0·000<0.05),Qingyi Decoction combined group decreased more significantly than the Magnesium Sulfate group(Finteraction= 1.997,Pinteraction=0.000<0.05).Total blood bilirubin in 24h after,3 days,7 days after admission showed a decreased trend in two group(Ftime=42.34,Ptime=0.000<0.05),but the difference between the two groups was not statistically significant(Finteraction=2.321,Pinteraction=0.103>0.05).3.There were significant differences in abdominal pain relief time between two groups(P<0.05);the abdominal pain remission rate in Qingyitang combined treatment group in 6 days was higher than the Magnesium Sulfate group;the remission rate of abdominal distension in 3 days in Qingyitang combined treatment group is higher than Magnesium Sulfate group.The two groups of patients admitted to hospital within third days of alleviating symptoms of abdominal pain and abdominal distension symptom relief rate are the same.The relieve ingrate of abdominal distension within 3 days after admission in Qingyitang combined group is higher than Magnesium Sulfate group.4.The gastrointestinal decompression duration,eating time,hospitalization time between the two groups were statistically significant differences(P<0.05).The gastrointestinal decompression time,the eating time,and the hospitalization time in Qingyitang combined group were significantly decreased compared with Magnesium Sulfate group.5.The cure rate of two groups of patients,the difference was statistically significant(P<0.05);the improvement rate of the patients in the two groups,the difference was not statistically significant(P>0.05);the total effective rate of the patients in the two groups,the difference was statistically significant(P<0.05).Conclusions:1.The recovery effect of blood amylase and white blood cell count in Qingyitang combined group were more significant.2.The abdominal pain and abdominal distension remission time in Qingyitang combined group was reduced than Magnesium Sulfate group,the alleviating effects of abdominal pain and abdominal distension within 3 days after admission in Qingyitang combined group was better than Magnesium Sulfate group.3.The time of gastrointestinal decompression,the time of eating and shorten the length of hospital stay in Qingyitang group were significantly shorten compared with the Magnesium Sulfate group.4.The total effective rate of the Qingyitang combined group was higher than the Magnesium Sulfate group.The cure rate in the Qingyitang combined group was significantly higher than the Magnesium Sulfate group.5.It is a good therapeuical method of traditional Chinese medicine and western medicine for Qingyitang combined with MOSO4 in the treatment of biliary pancreatitis.
