Objective: To investigate the potential role of nuclear factor-kB(NF-kB) activation in acute lung injury induced by severe acute pancreatitis and also to observe alveoli macrophages activity,inflammatory cytokins and endotoxin in blood.Then using different medicines,such as QingYiTang Decoction, Dexamethasone, Santonstadine, Keffuding and the results may give us some new ideas for curing acute lung injury.Methods: 1.5% sodium deoxycholate was injected inversely into the common biliopancreatic duct of rat to improve the model of SAP, Sham operated controls(n=10); SAP model(n=10); QingYiTang Decoction(n=10); SAP+ Dexamethasone(n=10); SAP+ Santonstadine(n=10); SAP+ Keffuding(n=10). All the rats were killed at 24 hours after operation.Pathological examination and some indexes of ALI were measured.Seperated and purificated the alveoli macrophages from healthy pure made SD rats,cell concentration 5 105 ,and inoculated into the 24 hole culture medium.Then divided into Controlling group,StimuIating group ( LPS 10 g/ml) , QingYiTang Decoction group (LPS+ QingYiTang Decoction) , Dexamethasone group (LPS+ Dexamethasone) .After incubating 3h, 5h, 6h and 24h respectively ,we determined the NF-kB activity and the amount of iNOSmRNA in alveoli macrophages by electrophoretic mobility gel-shift assay and reverse transcription-poly merase chain reaction (RT-PCR) ,we also measured the level of NO, TNFa, iNOS,etal.Results: The level of NF-kB, iNOSmRNA, NO, TNFa, iNOS,ET,Amylase in AMs and serum of model group were significantly higher than those of sham operated control group(p<0.01).Some other indexes of ALI showed obvious pathogenic changes in model group and so did the pathogenic examination of lung tissue. QingYiTang Decoction, Dexamethasone, Santonstadine could decrease the NF-kB activity,reduceiNOSmRNA expression and eliminate the extent oflung injury ,but Keffiiding could not do that.The decline of ET, NO, TNFa in QYT decoction group was more significant. Santonstadine could inhibit amylase and inflammatory cell factor secreating by AMs obviously.And Dexamethasone showed superiority in ameliorating.After stimulating the seperated and purificated normal alveoli macrophages by LPS,the TNFa, NO in culture succus and the content of iNOS, iNOSmRNA, NF-kB in AMs were significantly higher than those in control group(p<0.01).Those in QYT decoction group, Dexamethasone group were obviously lower than stimulating group.Conclusions: During acute lung injury induced by severe acute pancreatitis, many factors have very important effects,such as NF-kB activity which start up the voluming expression of TNFo,iNOS and NO,excessive activation of AMs,a large amount of inflammatory factors,higher level of ET in blood,et al. QYT decoction could exert protecting effects through eliminating NF-kB activity,reducing excessive activation of AMs,alleviating the mass expression of TNFa.iNOS and NO,decreasing the higher level of ET and Amylase,et al. Dexamethasone and Santonstadine could realize their available effects in ALI by eliminating NF-kB activity,affecting some other aboving aspects.In a word,traditional and western medicine make very important effects in different phases during acute lung injury induced by severe acute pancreatitis.If associativly applying traditional and western medicine reasonably,we will find more useful methods to prevent and cure ALI induced by severe acute pancreatitis in clinic.
