Effect Of Tengqi Tongluo Granule On Expression Of IMA And Lp-PLA2 In Lower-extremity Arterial Disease Of Type 2 Diabetes Mellitus

Purpose:To evaluate the efficacy and safety of Tengqi Tongluo Granule in the treatment of diabetes mellitus patients with lower-extremity arterial disease,and explore the intervention of Tengqi Tongluo Granule on IMA and Lp-PLA2 levels.Method:By using the queue,parallel control research methods,60 cases of patients with DLEAD in qi deficiency and blood stasis syndrome were divided at random into treating group and the control group,each of 30 cases.The patients in the treating group were given oral administration of Tengqi Tongluo Granules based on the basic therapy(hypoglycemic,antihypertensive,lipid-lowering and anti-platelet aggregation),while the control group was given only basic therapy.After continuous medication for 12 weeks,we evaluate the chinese medicine symptoms;VAS scores,ankle-brachial index(ABI),glucose and lipids metabolism,micro-vascular complications index(urinary ACR);and expression levels of Lp-PLA2 and IMA.Results:1.Comprehensive efficacy:The total efficacy rate was 90.00%in the treating group and 60.00%in the control group.The curative effect of the treating group was better than that of the control group(P<0.05),thus the improvement of the coldness,tingling,and fatigue in treating group were better than those in the control group(P<0.05).2.IMA and Lp-PLA2:The decrease of IMA in treating group was14.68±1.34U/mL VS control group5.53 ± 1.85 U/mL,the treatment group was better than the control group(P<0.05).Lp-PLA2 in both groups was decreased after treatment(P<0.01 or P<0.05),the discrepancy before and after treatment in treating group was 24.26±4.31ng/ml VS control group 5.92±3.21ng/ml,but the therapeutic effect was significantly better in the treatment group than in the control group(P<0.01)3.Glucose and lipids:The levels of TC,TG,LDL-C in the treating group were signifi cantly decreased than those before treatment(P<0.01 or P<0.05),details as follows:TC:treating group 0.784±0.183mmol/L VS control group 0.227±0.158mmol/L;TG:treating group 0.784±0.308mmol/L VS control group 0.340±0.188mmol/L,LDL-C:treating group 0.241±0.164mmol/L VS control group 0.121±0.235mmol/L.There was no significant difference of glucose metabolism between the two groups before and after treatment(P>0.05)5.ABI:The levels of ABI in both groups were lower than those before treatment(P<0.01 or P<0.05),details as follows:treating(left side0.11±0.08/right side0.10±0.06)VS control group(left side 0.04±0.07/right side 0.03±0.06),but the curative effect of the treating group was significantly better than that of the control group(P<0.01)Conclusion:1.Tengqi Tongluo Granule has a better effect in treating DLEAD;and it has a significant effect in improving clinical symptoms,especially for coldness,pain and fatigue.2.Tengqi Tongluo Granule can reduce the level of IMA,Lp-PLA2;improve lipid metabolism,significantly reduce the TC,TG,LDL-C,and improve the ABI;it can also reduce lower limb ischemia,delay the occurrence of lower extremity vascular atherosclerosis.3.Tengqi Tongluo Granule had no adverse drug reactions and the security is good.
