Objective:In order to provide theory basis for reducing and preventing the CSP(caesarean scar pregnancy),the clinical data,such as patients age?physical condition?caesarean section?caesarean section uterine incision suture method and postoperative recovery were analyzed,the influence factors on the pathogenesis of CSP were discussed.Methods:In this study,51 patients with CSP inour hospital between January 2008 and October 2016 were selected as experimental group(CSP group);at the same time,59 cases with normal pregnancy after the caesarean section were selected as control group.The clinical data of both the experimental group and control group,including age,physical condition(body mass index,anemia,chronic diseases,pelvic operation and abdominal operation),uterine position,caesarean section condition(caesarean section reasons and vaginal delivery process),gestation hi story(pregnancy times,abortion times and caesarean delivery times),caesarean section uterine incision suture method,distance the previous caesarean section time and postoperative recovery were retrospective analyzed,respectively.Results:1?The comparison of age between CSP group and control groupThe caesarean section age and onset age of the patients in CSP group were quite concentrated,compared with the control group,there is no significant difference of the two groups in age distribution(P>0.05);2?The comparison of physical condition between CSP group and control groupThere are no correlations between the pathogenesis of CSP and the body mass index about pre-delivery before operation and the onset,the anemia situation in the caesarean section and the onset,chronic diseases,pregnancy complications,pelvic operation and abdominal operation,the differences between the two groups are not si gni fi cant(P>0.05).3?The comparison of uterine position between CSP group and control groupThere were 10 cases(19.6%)with the posterior uterus in the CSP group,higher than that in the control group,the difference between the two groups is statistically significant(P<0.05).4?The comparison of clinical symptoms between CSP group and control groupIn the CSP group,39 cases(76.47%)suffered the abdominal pain and vaginal bleeding in the early pregnancy period,which was higher than that in the control group,the difference between the two groups was statistically significant(P<0.05).5.The comparison of pregnancy times between CSP group and control groupIn the CSP group,45 cases of total pregnancy ? 3 times(88.24%),12 cases of caesarean section ? 2 times(25.49%),26 cases of abortion ? 2 times(50.98%),11 cases of abortion after caesarean section>2 times(21.57%),which were higher than that in the control group,there are significant differences between the two groups(P<0.05);5 cases of abortion before caesarean section ?2 times(9.8%),there is no significant differences between the two groups(P>0.05).6?The comparison of time interval from the previous caesarean section between CSP group and control groupIn the CSP group,the time intervals from the previous caesarean section were focused on 24-96 months(2-8 years),compared with the control group,the difference had not statistical significant(P>0.05).7?The comparison of caesarean section condition between CSP group and control groupIn the CSP group,10 cases of premature rupture of membranes(19.6%),compared with the control group,the difference is not significant(P>0.05);caesarean section of trial production in 5 cases(9.8%)and caesarean section of abnormal labor in 4 cases(7.8%),lower than that in the control group,the differences are significant(P<0.05);breech caesarean section in 13 cases(25.49%),higher than the control group,the difference is statistically significant(P<0.05);the cases with caesarean section resulted from oligohydramnios,macrosomia,without medical indications,hypertensive disorder complicating pregnancy,intrauterine infection and placenta previa were compared with that in control group,and the difference was not significant(P>0.05)8?The comparison of postoperative fever between CSP group and control groupThere are two patients(3.92%)fevered after caesarean section in the CSP group,compared with the control group,the difference is not significant(P>0.05);Conclusion:1?Most patients with CSP will suffer the abdominal pain and(or)vaginalbleeding in the early pregnancy period.2?The retroverted uterus is the related factor of CSP.3?Multiple caesarean section,abortion,especially the abortions after caesarean section were the related factors to the incidence of CSP.4?The breech and no caesarean section of trial production caesarean section were the related factors in the pathogenesis of CSP.
