Purpose:This study adopted case-control method,through investigation,oral examination and laboratory examination of patients with questionnaire,the relationship between gingival bleeding symptoms and long-term anticoagulant drugs,so as to provide a theoretical basis for the prevention and treatment of gingival bleeding symptoms.Method:Case control study.(1)the case group:a total of 32 patients who underwent coronary angiography and interventional treatment of surgery,postoperative combined application of aspirin and clopidogrel(for Grillo)more than half a year also complained of gingival bleeding and gingival bleeding on probing rate of BOP is greater than 50%of patients(28 males,4 females);(2)control group:we select the same number of coronary artery stent surgery,postoperative combined application of aspirin and clopidogrel(ticagrelor)more than half a year without symptoms of gingival bleeding,gingival bleeding on probing rate is less than 25%of patients(23 males,9 females).All the questionnaire and oral examination of two groups of subjects,the transfer of blood coagulation function and platelet aggregation function check inspection results,record all inspection data,and statistical analysis was performed using SPSS 17.0 software by category.Result:After equilibrium test,the data of two groups of patients have good comparability.Analysis of examination results of patients with oral group case group and control group,compared two patients with oral hygiene index,gingival index had significant difference(P<0.01),plaque index and sulcus bleeding index(<0.01P<0.05),there are some differences in the calculus index and soft scale index of two groups are different,but the difference was not statistically significant;Periodontal health status included gingival recession,tooth loosening,tooth loss,attachment loss and probing depth of periodontal pockets.The differences between the two groups were statistically significant(P<0.05).Comparative analysis of periodontal disease risk factors,smoking(OR=4.879,95%CI=1.406-1.657),hypertension(OR=6.176,95%CI=1.183-1.142)and scaling(OR=0.358,95%CI=1.405-1.656)consciousness had statistically significant difference(P<0.05),brushing frequency and brushing time difference,but no statistical significance.Check the transfer of data,laboratory analysis,preoperative coagulation function indexes of activated partial thromboplastin time(APTT)had significant difference(P<0.05);Platelet aggregation function analysis showed that the mean aggregation rate(AAR)and the maximum platelet aggregation rate(MAR)of two platelet phosphate(ADP)as the inducing agent were statistically different(P<0.05).Conclusion:The patients themselves poor periodontal status or suffering from periodontal disease and without formal treatment,and long-term use of anticoagulation drugs,showing symptoms of bleeding gums especially bleeding teeth,even spontaneous bleeding;patients with periodontal health long-term regular oral anticoagulation drugs did not appear obvious symptoms of bleeding gums.Good brushing habits and oral health awareness,adhere to every 1-2 years for a dental patients with gingival bleeding symptoms were significantly reduced.For the treatment of gingival bleeding symptoms of the origin is the treatment of periodontal disease,control of periodontal tissue inflammation,patients also do their own cleaning and maintenance,alleviate the symptoms of bleeding gums,periodontal disease control.Survey findings,the majority of male patients with coronary heart disease and smoking history,alveolar bone absorption,tooth loosening,but gingival bleeding symptoms are not obvious.
