Objective:To analyse the clinical features of abdominal infections carbapenem-resistant Klebsiella pneumoniae in patients with risk factors and mortality at 30 days.Methods:A retrospective analysis of the clinical features of risk factors and mortality at 30 days of patients with abdominal infections in carbapenem-resistant Klebsiella pneumonia.All the patients were hospitalized at the First affiliated Hospital of Zhejiang University from February 2013 to April 2014.Inclusion criteria is the patients which cultured carbapenem-resistant Klebsiella pneumoniae in peritoneal drainage fluid.Patients younger than 10 years old and with incomplete clinical data were excluded.Data were analysiesd by univariate analysis,chi-square test and risk assessment.Results:In this study,there were 25 cases of abdominal infection which were not sensitive to carbapenems alkenes of Klebsiella pneumoniae(n = 25)with a mean age of 50.08 years(SD,14.92);Male accounted for 76%.The APACHE II score:27.16±3.23.68%of patients had been admitted to ICU;Surgical treatment within 90 days before cultured positive were 84%;80%of patients had received antibiotic treatment in two weeks before a positive culture;All patients had taken over invasion operations was two weeks(peritoneal drainage tube or catheter 100%;central venous tube 96%;intubated 76%);patients which had blood or lower respiratory infections accompanied by CRKB was accounted for 40%and 20%respectively;fever is the most common clinical manifestations(64%),while abdominal pain was 40%;complicated by septic shock is 32%;culture results were positive in all patients after herein are antibiotics.Recent mortality rate was 32%(within 30 days).Septic shock was the only risk factor for mortality within 30 days(OR:6.936,95%CI(1.591-30.245),P=0.010).ConclusionThe patients which had abdominal infections with CRKB showed higher mortality.Septic shock is the unique risk factor for mortality within 30 days.
