Outcome Of In Vitro Fertilization In Endometriosis-associated Infertility

ObjectiveTo discuss the pregnancy outcome of endometriosis-associated infertility after in vitro fertilization.MethodsA retrospective analysis of the endometriosis-associated infertility from January 2010 to December 2013 after in vitro fertilization including the history of infertility,the laboratorial data and the pregnancy outcome data.Results1.Endometriosis stage ?-? and ?-? primary infertility rate were significantly higher than fallopian tube following infertility(P<0.01);within the stage ?-?required more Gn,FSH,HMG than stage ?-?(P<0.05),which also had lower oocytes retrieved number(P<0.05),longer days of Gn(P<0.05),lower fertilization rate but no significant statistical difference(P>0.05).The E2 levels after HCG injection were significantly reduced(P<0.05).2.The dosage of Gn in ovarian endometrioma was higher than that of peritoneal endometriosis and mixed endometriosis(P<0.05).The E2 levels after HCG injection of ovarian endometrioma was lower than that of peritoneal endometriosis and mixed endometriosis(P<0.05).Ovarian endometrioma had the highest numbers of retrieved oocytes(P<0.05),but peritoneal endometriosis had the highest fertilization rate(P<0.05).3.Ultra-long downregulation protocol group required more Gn,FSH,HMG(P<0.05)compared with long agonist protocol,the Gn days were longer but no statistical difference(P>0.05),the E2 levels after HCG injection were lower(P<0.01)and had lower oocytes retrieved number(P<0.05).Ultra-long downregulation protocol group clinical pregnancy rate was higher(P<0.05).4.Results showed that the incidence rates of placenta abnormalities were higher in the long agonist protocol group than fallopian tube following infertility(P<0.05).ConclusionEndometriosis stage ?-? required more drugs than stage ?-? and had lower oocytes retrieved number,lower fertilization rate HCG injection day E2 were significantly reduced.At the same time,for endometriosis-associated infertility patients,ultra-long downregulation protocol might increase the use of drugs,but it could improve the rate of pregnancy and pregnancy outcome.
