Clinical Observation Of Osteonecrosis Of The Femoral Head In Children With Oral Liquid Of Gubao

Objectives:To explore the therapeutic effect of Oral Liquid of Gubao(OLG)with hip abduction orthosis in treating osteonecrosis of the femoral head in children,and to provide the clinical basis for the treatment in children with earely avascular necrosis of the femoral head,the clinical curative effect was observed and the the clinical effect was evaluated objectively.Methods:Randomized single blind controlled design was used in this study,80 patients with avascular necrosis of the femoral head(Perthes Disease)who met the inclusion criterias were chosed in Foshan City Hospital of traditional Chinese medicine from March 2015 to January 2017.According to the random number table method of 1:1,all the patients were randomly divided into the treatment group and the control group,40 cases in each group.The control group was treated only with hip abduction brace,while the control group was treated with OLG and hip abduction orthosis.Every 3 months were defined as one course,and there were 3 courses in our treatment.Before and after the treatment,VAS method was used to evaluate the relief of pain,the clinical symptoms and signs were compared by using scoring method,and the Mckay standard was used to evaluate the hip function of the two groups.Results:The comparison of general data;40 cases were in the control group,including 29 male and 11 female,the average age was 5.48±1.340 years,40 cases were in the treatment group,including 32 male and 8 female,the average age was 5.58 ±1.394 years;There was no significant difference between the two groups in sex,age and Herring classification(P>0.05).The experimental results showed:?Before the treatment in the two groups,the VAS score was no significant difference,both two groups had the significant difference in the treatment of pain(P<0.01),the treatment group had the better results than the control group(P<0.05);?Before the treatment in the two groups,the scores of clinical symptoms and signs were no significant difference,after treatment both clinical symptom and syndrome scoring markedly decreased(P<0.01)in the two groups,and the treatment group had better results than the control group(P<0.05),showing significant difference between the two groups statistically,especially in the improvement of the hip joint activity;?The Mckay score of hip joint,there was statistical significance after treatment in two groups(P<0.05),and the excellent rate of the treatment group(77.5%)was higher than the control group(57.5%),in improving the hip joint function,the use of OLG in children with hip abduction orthosis significantly better than those who only wearing hip abduction orthosis(P<0.05).Safety:During the clinical experiment,there were 5 cases of oppression and discomfort on articular prominents,due to the delay of referral,there was 1 case with pressure ulcer on the joint prominents,after brace adjustment and wound dressing,all symptoms disappeared;After all the treatment,there were no nausea or vomiting,no abdominal pain diarrhea or other significant drug reactions of OLG.Conclusion:Compared with pure hip abduction orthosis treatment,treatments using OLG combined with hip abduction orthosis for ischemic necrosis of femoral head in children was better in the relief of pain and the improvement of clinical symptoms and signs as well as the recovery of hip function.Using OLG combined with hip abduction orthosis is safe,reliable and worthy for clinical application.
