Aims:To assess the actual health problems and the dynamic changement of the knowledge,behavior and status of each actual health problem among the disabled elderly under the intergrated service model of healthcare.Methods:A principle of convenience sampling was used.A total of 80 disabled elderly under the intergrated service model of healthcare in a community health center in Hangzhou were assessed based on an international standardized nursing language(Omaha System)for the actual health problems and the state of knowledge,behavior and status of each health problem.The first assessment was carried out when the disabled elderly just signed for the intergrated service model of healthcare.Then the follow-up assessment was assessed every two months,each assessment was about one hour.The assessment duration lasted for six months.An open question(what were your major health problems according to your own situation)was asked to make the assessment more comprehensive at the end of assessment.Descriptive analysis(mean,standard deviation,frequency,composition ratio)was used to describe the general information,actual health problems,the knowledge,behavior and status of each health problem.Friedman test was used to analyze the dynamic changement of the knowledge,behavior and status of each health problem.Results:The disabled elderly had 12.7±3.13 health problems at the first assessment,12.3±2.89 health problems at the second assessment,12.2±2.74 health problems at the third assessment,and 12.2±2.75 health problems at the fourth assessment.During the follow-up assessment,the common health problems(the problems thatincidence rate were 50% or higher)among the disabled elderly were social contact,mental health(psychosocial domain),vision,oral health,cognition,neuro-musculoskeletal function(physiological domain),physical activity and personal care(healthrelated behaviors domain).The knowledge of social contact,mental health,oral health and circulation of the disabled elderly were significantly increased with time(P< 0.05).The behavior of circulation was significantly increased(P< 0.05),and the status of social contact,neuro-musculo-skeletal and circulation of the disabled elderly were significantly increased with time(P< 0.05).Conclusions:The disabled elderly in community had certain health problems(an average of about 13 health problems in the 42 health problems).Among the common health problems,the behavior and status of each common health problem was lower than the knowledge.The intergrated service model of healthcare might improve the knowledge of social contact,mental health,oral health and circulation,the behavior of circulation and status of social contact,neuro-musculo-skeletal and circulation among the common health problems of the community-dwelling disabled elderly.While the improvement of other health problems of the disabled elderly under the intergrated service model of healthcare has not yet been found.
