Treatment Of Type C Fractures Of Distal Radius With Volar Locking Compression Plate And Fracture Of The Third Phase Of TCM Syndrome Differentiation

Objective : To investigate the outcomes,operation skills and the matters needing attention of type C distal radial fractures treated with volar locking compression plate and fracture of the third phase of TCM syndrome differentiation.Methods : Collected from December 2014 to June 2016 using volar locking compression plate combined with distal radius fractures in the third phase of TCM syndrome differentiation treatment of type C fractures of the distal part of the radius of56 cases.Which uses the pure radial distal volar Locking compression plate(Locking compression plate,LCP)fixed group of 28 cases(control group),7 patients were male,female 21 cases;Aged 39-80,the average(46.64 ±15.49);Left 11 cases,17 were right;According to the AO/ASIF classification: C1 type fractures in 9 cases,11 cases of type C2,fractures of type C3 in 8 cases;merge the ulna styloid process fracture in 3 cases.Using volar locking compression plate combined with distal radius fracture three periods of TCM syndrome differentiation treatment(LCP + fracture 3 period syndrome)group(treatment group),28 cases,9 cases of male and female 19 cases;37-82 years old,average(45.54 ± 45.54);19 cases left,9 cases right;According to the AO/ASIF classification: C1 type fractures in 10 cases,12 cases of type C2,fractures of type C3 in6 cases,combination of ulna styloid process fractures in 4 cases.All of the patients with volar approach and locking compression plate internal fixation of distal radius,fracture treatment group and the third phase of syndrome differentiation,early for the therapeutic principle for promoting blood circulation to remove blood stasis,acetanilide detumescence,with Taohong siwu decoction,the formulas of mid-to renew and camp pain,bone muscle method,analgesic tonga Chang Yihe camp continued bone huoxue soup with syndrome differentiation,late to benefit gas blood,liver kidney,dredging channels for treatment,raw pulp and cutting with blood tonic soup,etc.Compare two groups of patients with fracture healing time,postoperative complications,wrist joint function score and volar tilt,radial inclination,radial height.Results:All patients were followed up,and the control group were followed up for 6 ~18 months,an average of 11±3.2 months.Treatment group follow-up time 5 ~ 19 months,an average of 12±4.5 months.All achieves osseous healing of fracture,the control group 7 to 11 weeks fracture healing time,average 9.0±0.5 weeks;Treatment group fracture healing time 6-10 weeks,an average of 7.5±0.8 weeks,the differences between the two groups have statistical significance(P < 0.05).Treatment group with no infection,delayed union,malunion,back extensor tendon irritability symptoms,carpal tunnel syndrome,complications,such as internal fixation loosening,the control one incision hematoma,after dressing the healing;one carpal tunnel syndrome,raise the control such as nerve improved after treatment.Two groups of patients volar tilt,radial inclination,radial height,there was no significant difference incidence of postoperative complications of significance(P > 0.05).According to the modified Gartland-Werley Sarmiento function score: control group 10 cases excellent,10 cases good,8 cases fair,with an excellent and good rate of 71.43%;Treatment group(16 cases excellent,11 cases good,1 cases fair,the excellent and good rate of 94.43%.)The difference between the two groups was statistically significant(P < 0.05).Conclusion:Using locking compression plate combined with distal radius fracture the third phase of TCM syndrome differentiation treatment with volar approach of type C fractures of the distal part of the radius,the fracture,postoperative fracture maintain to fracture healing,fracture healing time is short,less postoperative complications,in the interest of early functional exercise,postoperative function recovery,worthy of being widely applied in clinic.
