| Objective: 1、Discuss the etiology and pathogenesis of ISR,from the perspective of TCM.The traditional Chinese medicine intervention should be based on the principle of promoting blood circulation to remove blood stasis and Tonifying Qi and blood;2.To study the effect,mechanism and prognosis on the prevention of In-stent Restenosis after cerebral vascular stent implantation in the Traditional Chinese and Western medicine intervention therapy that Taohong Siwu Decoction Combined with dual antiplatelet drugs.And we also observe the security of this scheme.Methods: 60 patients with cerebrovascular stent implantation in our hospital had been chosen to support our research,and everyone be numbered according to the time sequence of patients of our hospital(1-60).Through the method of randomly divided into two groups,a treatment group and a control group.That is 30 cases in each group.The control group in compliance with "Chinese ischemic cerebrovascular disease interventional diagnosis and treatment guide 2015" [1] will be treamted with dual antiplatelet drugs(aspirin and clopidogrel aspirin 0.1g QD,75 mg QD)6 months of treatment.The treatment group in the dual antiplatelet treatment combined with Taohong Siwu Decoction(in order to strengthen the compliance of the patients,treated with the granules not decotion.)It will take 6 month course in close follow-up.We will observe adverse reactions after surgery,detection of coagulation,NIHSS score,and review DSA 6 months after surgery,evaluate the stent and vascular lesions after operation will be analyzed,and then analyzed the effectiveness of the intervention of ISR.Results: 1,.This study collected a total of 60 cases of patients with follow-up.In a total of 6 months of follow-up,2 cases of the treatment group got lost.1 case refused to review the DSA.1 case of the control group got lost and 1 case refused to review of DSA.There aren’t any failure operation cases in both two groups.Overall,in 60 cases,a total of 5 patients were not followed up in the follow-up.The final completion of follow-up cases in 55 cases,the treatment group of 27 cases(90%),the control group(n = 93.3%),the follow-up rate of the two groups was not statistically significant.2.There was no significant difference in gender,age,course of disease and risk factors between the two groups.3.There was significant difference in the incidence of ISR between the two groups(p<0.01),the incidence of ISR in the treatment group was 7.41%,and that of the control group was about 25%.4.There was no significant difference in the NIHSS score between the two groups before treatment.There was statistical difference between the two groups before and after treatment,and the NIHSS score of the treatment group was significantly lower than that of the control group after treatment NIHSS.5.Safety evaluation: two groups of patients before and after treatment and examination of blood and urine,feces and blood routine test,liver and kidney function.The results were within the reference range fluctuations in medicine,and no obvious change before and after the treatment.All the cases did not have patients with gastrointestinal bleeding,skin purpura and other mucosal bleeding.Conclusion: The treatment of Taohong Siwu Decoction Combined with dual antiplatelet drugs has better curative effect in the prevention of ISR than simple application of antiplatelet drugs.And it can improve the prognosis of patients.Although Taohongsiwu Decotion promots blood circulation of the power,the accumulation of drug prevention of ISR in combined dual antiplatelet therapy is safe and effective.there is no obvious adverse reaction. |
