THEORETICAL RESEARCH:Diabetic nephropathy is one of the most serious complications of diabetes,and is also the main cause of end-stage renal disease.However,modern medicine for the pathogenesis of diabetic nephropathy has not been fully elucidated,and there is no effective treatment and medicine.Currently,the importance of the mechanism of podocyte injury concern,especially podocyte membrane associated protein--nephrin and podocin could reduce the kidney damage,it provides a new way to seek a better way for the treatment of diabetic nephropathy.DN belongs to the category of traditional Chinese medicine"Xiao Ke""Shen Xiao""Xia Xiao"and so on.TCM has certain characteristics and advantages in the treatment of this disease.JiaWeiFuShenTang(JWFST)is based on the accumulation of clinical practice for many years,and has been modified by the effective FuShenTang of diabetic nephropathy before.JWFST groups with diabetic nephropathy " Qi deficiency and blood stasis,the context of kidney is not smooth",the characteristics of the formula reflects its multiple system symptoms,multiple organs were cast and multi targets drugs.So JWFST is closer to clinical reality than FST,and it is worthy of further study for.EXPERIMENTAL STUDY:OBJECTIVE:To observe the effects of JiaWeiFuShenTang on diabetic nephropathy rats with renal lesions and discuss the possible mechanism;METHODS:The normal control group(C)was 8 male Wistar rats with conventional feeding;32 male Wistar rats were treated with intraperitoneal injection of STZ+CFA to establish diabetic model after normal feeding for 1 weeks.After successful modeling,the rats were divided into model group(M),JWFST(L1)group,JWFST(L2)group,JWFST(L3)group.L1,L2,L3 group were treated with JWFST 8mg/(kg·d),16mg/(kg · d),32mg/(kg · d);the model group was given normal saline.The weight,blood sugar on a regular basis for 8 weeks were recorded.After the experiment,the serum and urine were collected and biochemical parameters,like INS,TG,CHO,LDLC,HDLC,SCr,BUN,SOD,MDA,AST,ALT and UP were detected.Then the kidneys were harvested and the expressions of nephrin and podocin in the kidney were detected by IF method,RT-Qpcrmethod and Western blotting.And then observed the renal lesions under light microscope.RESULTS:Compared with the model group,the INS,TG,CHO,HDLC,SCr,BUN,MDA,AST,ALT,UP of treatment groups were significantly decreased,and LDLC,SOD was increased significantly.The expressions of nephrin and podocin were raised significantly by JWFST.ONCLUSION:?JWFST could significantly reduce the pathological scores,improve the animal model of mesangial proliferation,extracellular matrix(ECM)accumulation,glomerular atrophy and expansion and other pathological manifestations.Finally,the renal damage of DN rats reduced.? JWFST did not significantly reduce the role of blood glucose,it can be seen that the role of DN to improve renal damage and the improvement of blood glucose is not related.?JWFST had obvious effects on reducing the MDA and increasing the SOD of DN rats.These results suggest that JWFST plays an important role in improving the renal pathological changes in DN rats,which may be related to the reduction of oxidative stress and the reduction of ROS production.?JWFST could reduce SCr,BUN to improve renal function,and reduce ALT,AST to improve liver function.It is well known that DN is mainly located in the kidney,but the so-called "liver and kidney homology" in traditional Chinese medicine,it suggests that they are closely related to each other.?JWFST could improve the expression of nephrin and podocin protein in the kidney of DN rats,which can help to improve the podocyte injury of DN rats,help to repair the glomerular filtration barrier and alleviate the renal damage in DN rats.The composition of JWFST has certain basis.It can alleviate the lesion of kidney in DN rats,protect the kidney,and the important mechanisms may be improving the podocyte injury,reducing the oxidative stress and reducing the formation of ROS in DN rats.
