Objective: To evaluate the risk of stiffness between early and delayed ACL surgery in studies that use a modern accelerated rehabilitation protocol, and to analyze the TT-TG distance between patients with ACL injuries and control group, and the preoperative and postoperative variation. Methods: Retrospective analysis was adopted to analyze the TT-TG distance measured on MRI.29 cases of reconstruction of ACL and 33 cases of control group from February 2011 to December 2013 were included. With at least one year follow-up,the knee functional scales and stability examinations were conducted for the postoperative assessment.Measurement data, count data and level data were analyzed by independent sample t-test,chi-square test and Wilcoxon Rank Sum Test separately. Results: No significant increase in risk of postoperative stiffness with early surgery using a variety of cutoff points.TT-TG distance of the injured knee was significantly lower than the preoperative(P<0.05),while there were no differences compared with the preoperative, postoperative contralateral knee and the control group(P>0.05);With the comparison of the preoperative and postoperative, there was significant difference in the Lysholm scores,IKDC grade classification and stability examination(P<0.05).Conclusion: If a modern surgical technique and an accelerated rehabilitation protocol are used, there is no increased risk of knee stiffness if an ACL reconstruction is performed as early as 1 week after injury.Preoperative TT-TG distance of ACL injured patients was higher than the contralateral knee and the control group, the postoperative TT-TG distance of ACL injured patients was decreased to normal level, which indicated ACL reconstruction could adjust the abnormal kinematics caused by injury, restore the tibiofemoral joint status, and prevent knees joint from abnormal articular cartilage contact and osteoarthritis. However, future studies are needed on this issue to provide more evidence.
