Purpose:The left ventricular and right ventricular function changes in subclinical hypothyroidism patients were evaluated by two-dimensional Speckle tracking imaging,and their cardiac function was found early.Materials and methods:Subclinical hypothyroidism group(SCH): 34 patients have been diagnosed as subclinical hypothyroidism during January 2016 to December 2016,(female 23,male 11,age 35-50 years,mean age 40.2 ± 1.6 years).Normal control group: 20 healthy volunteers(female 14,male 6,age 35-45 years,mean age39.9 ± 2.8years)were recruited.Each group of patients underwent routine echocardiography under resting condition,and two standard gray-scale dynamic images of six standard sections(the left ventricle mitral valve short axis,the left thoracic papillary muscle minor axis,Chamber apex short axis and apical four chamber section,apical two chamber section,apical three chamber section),frame rate greater than 100 frames / sec,each section recorded more than 3 cardiac cycles.Left ventricular end-diastolic diameter(LVIDd),left ventricular end-diastolic diameter(LVIDs),diastolic ventricular septal thickness(IVSd),end-diastolic left ventricular posterior wall thickness(LVPWd),right ventricular diastolic(RVIDd),right ventricular end-diastolic diameter(RVIDs),right ventricular end-diastolic area(RVEDA)and right ventricular end-systolic area(RVESA)were used to calculate the right ventricular area change rate(RVFAC)Left ventricular ejection fraction(LVEF).Thyroid annulus systolic displacement(TAPSE)was measured using M-mode ultrasonography.The left ventricular global longitudinal strain at systole(GLSs),global circumferential strain at systole(GCSs),longitudinal strain rate(LRS),circumferential strain rate(CRS)and the right ventricular global longitudinal strain at systole(GLSs),longitudinal strain rate(LRS)were measured and analyzed by using QLAB 9.0 software CMQ postprocessing images.The TMAD midspat was used to measure tissue motion annular displacement at the midpoint(TMAD midpt)and the ratio of tissue motion annular displacement at the midpoint(TMAD midpt%).Results:(1)SCH group compared with the control group in generalSCH group compared with the control group,age,sex,triiodothyronine(FT3),Thyroxine(FT4)was not significantly different,no statistically significant(P> 0.05).Thyroid hormone(TSH),heart rate,blood pressure and the control group compared with the difference was statistically significant(P <0.05).(2)Comparison of routine ultrasonography and TMAD between SCH group and control groupThere was no significant difference in LVIDd,LVIDs,IVSd,LVPWd,RVIDd,RVIDs,RVEDA,RVESA,RVFAC,LVEF and TMAD between the SCH group and the control group(P> 0.05).However,the difference between Midpt and TMAD Midpt% was statistically significant(P <0.05).(3)Comparison of left ventricular myocardial strain and strain rate between SCH group and control groupCompared with the control group,the GLSs and GCSs of the left ventricle decreased compared with the control group,the difference was statistically significant(P <0.05).(LRSe,LRSa,LRSs)were significantly lower than those in the control group(P <0.05),and the difference was statistically significant(P <0.05).(CRSe,CRSa,CRSs)were also lower than those of the control group(P <0.05).(4)Comparison of right ventricular ventricular strain between SCH group and control groupCompared with the control group,the GLSs of the right ventricle and the peak strain rate(LRSe,LRSa,LRSs)in the early systolic,diastolic and diastolic dilatation were significantly lower than those in the control group(P <0.05).Conculsions:1.Conventional echocardiography can not assess subclinical hypothyroidism in patients with early dysfunction to heart function.2.Two-dimensional speckle tracking imaging technique can detect subclinical hypothyroidism in patients with early left ventricular systolic and diastolic dysfunction,for clinical and timely intervention to provide a reliable basis.3.Subclinical hypothyroidism in patients with right ventricular systolic and diastolic dysfunction.
