Objective:To investigate the function of Th1,Th2 cells and relevant cytokines in the progress of HT,PTC and HT with PTC and the correlation between HT and P TC.Methods:Twenty-four patients with HT,twenty-four patients with PTC,twenty-one HT patients with concomitant PTC and Twenty controls were included in our present study.The proportion of Th1、Th2 cells in peripheral blood were detected by flow cytometry.In addition,ELISA was uesd to test the levels of Th1、Th2 relevant cytokines in serum.Results:1.The proportion of Th1 cells in peripheral blood of HT patients(26.92±6.83)and HT patients with concomitant PTC(26.30±4.47)was higher than that of the control group(21.34±7.02)(P=0.011,0.019),and the proportion of Th1 cells in peripheral blood of PTC patients(18.09±4.29)had no statistical differenc e to the control group(P=0.081);The proportion of Th1 cells in peripheral blood of PTC patients was lower than that of HT patients and HT patients with concomitant PTC(P=0.000,0.000);The proportion of Th1 cells in peripheral blood of HT patients with concomitant PTC had no statistical difference to HT patients(P=0.426).The proportion of Th2 cells in peripheral blood of HT patients(1.64±0.54),PTC patients(2.10±0.59)and HT patients with concomitant PTC(1.78±0.65)had no statistical difference to the control group(P=0.074,0.200,0.446);The proportion of Th2 cells in peripheral blood of PTC patients was higher th an that of HT patients(P=0.007);The proportion of Th2 cells in peripheral blood of HT patients with concomitant PTC had no statistical difference to HT patients and PTC patients(P=0.456,0.085).The ratio of Th1/Th2 in the HT patient s(17.40±5.54)and HT patients with concomitant PTC(16.13±4.59)was higher than that in the control group(11.04±2.58)(P=0.000,0.001),and the ratio ofTh1/Th2 in the PTC patients(8.86±1.79)was lower than that in control group(P=0.003);The ratio of Th1/Th2 in the PTC patients was lower than that in the HT patients and HT patients with concomitant PTC(P=0.000,0.000);The ratio of Th1/Th2 in the HT patients with concomitant PTC had no statistical difference to HT patients(P=0.412).2.The expression of IFN-γ in serum of HT patients(4.41±1.45)and HT patients with concomitant PTC(4.17±0.90)were higher than those of control group(3.18±1.83)(P=0.017,0.038),and the expression of IFN-γ in serum of PTC patients(2.26±1.13)had no statistical difference to the control group(P=0.062);The expression of IFN-γ in serum of PTC patients were lower than those of HT patients and HT patients with concomitant PTC(P=0.000,0.000);The expression of IFN-γ in serum of HT patients with concomitant PTC had no statistical difference to HT patients(P=0.500).The expression of IL-2 in serum of HT patients(3.04±1.35)and HT patients with concomita nt PTC(3.94±0.90)were higher than those of control group(3.07±1.59)(P=0.034,0.042),and the expression of IL-2 in serum of PTC patients(2.25±0.94)h ad no statistical difference to the control group(P=0.106);The expression of IL-2 in serum of PTC patients were lower than those of HT patients and HT patients with concomitant PTC(P=0.000,0.000);The expression of IL-2 in serum of HT patients with concomitant PTC had no statistical difference to HT patients(P=0.753).The expression of IL-4 in serum of HT patients(2.01±1.24),PTC patients(2.91±1.44)and HT patients with concomitant PTC(2.36±1.34)had no statistical difference to the control group(2.38±1.06)(P=0.172,0.181,0.947);The expression of IL-4 in serum of PTC patients was higher than that of HT patients(P=0.030);The expression of IL-4 in serum of HT patients with concomitant PTC had no statistical difference to HT patients and PTC patients(P=0.306,0.190).The expression of IL-10 in serum of HT patients(2.61±1.34),PTC patients(3.52±1.49)and HT patients with concomitant PTC(2.89±1.29)had no statistical difference to the control group(2.98±1.19)(P=0.134,0.234,0.696);The expression of IL-10 in serum of PTC patients was higher than that of HT patients(P=0.022);The expression of IL-10 in serum of HT patients with concomitant PTC had no statistical difference to HT patients and PTC patients(P=0.351,0.191).3.The proportion of Th1 cells,the ratio of Th1/Th2 and the expression of IFN-γ and IL-2 in peripheral blood of HT patients were positively correlated to TPOAb(r=0.607,P=0.002)(r=0.525,P=0.008)(r=0.588,P=0.003)(r=0.505,P=0.012).4.The ratio of Th1/Th2 in peripheral blood of PTC patients were associated with lymph nodes metastasis,and had nothing to do with size of lump,multifocal focal and capsule invasion.5.The proportion of Th1 cells,the ratio of Th1/Th2 and the expression of IFN-γ and IL-2 in peripheral blood of HT patients with concomitant PTC had nothing to do with size of lump,multifocal focal,capsule invasion and lymph nodes metastasis.Conclusions:1.Th1/Th2 balance shifted to Th1 exist in patients with HT,suggesting that the up-regulation of Th1 immune response may be associated with autoimmune damage.2.Th1/Th2 balance shifted to Th2 exist in patients with PTC,suggesting that the relative down-regulation of Th1 immune response and the relative up-regulation of Th2 immune response may be related to the progress of PTC.3.Th1/Th2 balance shifted to Th1 exist in HT patients with concomitant PTC,with the Th1/Th2 balance deviation between HT patients and PTC patients being opposite,and the correlation between HT and PTC is unclear.4.The proportion of Th1 cells,the ratio of Th1/Th2 and the expression of IFN-γand IL-2 may be one of the indicators of diagnosis and disease assessment.5.The ratio of Th1/Th2 is associated with lymphnode metastasis in PTC patients,which may be valuable for preoperative assessment of lymphnode metastasis.
