Clinicopathological Characteristics And Survival Analysis Of Different Sub-classifications Of Mucinous Breast Carcinoma

Purpose: To explore whether each sub-classification of MBC have the same prognosis,whether the individualized therapeutic strategies should be developed based on the sub-classification.Methods: We conducted a retrospectively analysis of the 136 cases of patients with MBC who underwent surgical treatment at the First Hospital of Jilin University between May 2005 and December 2015.2062 cases of patients diagnosed with IDC-NOS in the same period were set up as a control group.The clinicopathological characteristics differences were compared between the two groups.And the survival difference between the sub-classification of MBC and IDC-NOS were compared.Results: Compared with IDC-NOS,MBC had earlier pathological stage,higher tumor diameter,lower lymph node metastasis rate,higher ER,PR positive rate,lower Her-2 positive rate,higher Ki-67 low expression rate and better prognosis.PMBC had earlier pathological stage,lower lymph node metastasis rate and histological grade than MMBC.More of PMBC received breast-conserving surgery and radiotherapy,and less of them received chemotherapy than MMBC.The 5-year DFS of IDC-NOS,MMBC,PMBC-B and PMBC-A were 85.9%,85.2%,96.8%,100%,respectively,and the 5-year OS of them were 89.8%,89.8%,100% and 100%,respectively.PMBC had a better DFS and OS rate than IDC-NOS(P=0.006 and P=0.009,respectively).PMBC had a better DFS and OS rate than MMBC(P=0.009 and P=0.015,respectively).PMBC-A has a better DFS and OS rate than IDC-NOS(P=0.015 and P=0.035,respectively).PMBC-A has a better DFS and OS rate than MMBC(P=0.013 and P=0.039,respectively).There was no significant difference in DFS and OS between PMBC-B and MMBC,IDC-NOS.The DFS and OS rates between MMBC,PMBC-B and PMBC-A patients had a significant difference(P= 0.016 and P=0.028,respectively).In stage-matched analysis for DFS,MBC showed a better survival rate than IDC-NOS(P=0.048).In age-matched analysis for DFS,MBC showed a better survival rate than IDC-NOS(P = 0.041)in the patient group whose age was 51 or younger.Conclusions:1.Compared with IDC-NOS,MBC had earlier pathological stage,higher tumor diameter,lower lymph node metastasis rate,higher ER,PR positive rate,lower Her-2positive rate,higher Ki-67 low expression rate,but there was no signification difference in prognosis between them.2.Compared with MMBC,PMBC had earlier pathological stage,lower lymph node metastasis rate and histological grade.More of PMBC received breast-conserving surgery,and less of them received chemotherapy and radiotherapy.The prognosis of PMBC was better than MMBC.3.Compared with IDC-NOS and MMBC,the prognosis of PMBC-A was better than that of them.However,there was no significant difference in prognosis between PMBC-B,IDC-NOS and MMBC.4.TNM stage and ER status are independent prognosis factors of DFS in patients with MBC.Lymph node status,ER status and Age are independent prognosis factors of OS in patients with MBC.Therefore,not all MBCs should be regarded as a histological type with a favorable prognosis.Appropriate treatment strategies should be developed based on the sub-classification of MBC.
