objective:Coronary collateral circulation is critical for patients with coronary artery disease,as it can protect against myocardial tissue damage and improve the prognosis by compensating for blood flow deficits caused by relevant stenosis of major arteries.Inflammation plays an important role in the process of coronary collateral circulation..For example,vascular endothelial adhesion factor VCAM-1 can attract inflammatory cells to promote endothelial activation,and thus promote angiogenesis.Recent studies have shown that micro RNAs also play an important role in the regulation of angiogenesis and collateral circulation.n this study,the relationship between the formation of collateral circulation of coronary artery and the expression of micro RNA-155 and VCAM-1 in plasma was studied to evaluate the ability of micro RNA-155 and VCAM-1 to predict the collateral circulation of coronary artery,and to explore whether micro RNA-155 can be used as a biomarker of coronary collateral circulation.Method:A total of 112 cases were enrolled,the experimental group of 78 cases were enrolled from December 2015 to July 2016,all patients who underwent coronary angiography in the Department of Cardiology in our hospital for Cardiovascular Diseases,and who had at least one major coronary occlusion or a stenosis of ? 90%were screened for eligibility.At the same time,the experimental group was subdivided into two subgroups according to the Rentrop classification of collateral circulation evaluation system:Rentrop classification of grade 2 and 3 for the good group of coronary collateral circulation,n = 34 cases;Rentrop grade 0 and grade 1 were coronary collateral circulation failure group,n = 44 cases.In the control group,34 healthy subjects without coronary heart disease were collected at the same time period.The level of VCAM-1 in plasma was detected by ELISA,and micro RNA-155 was detected by real-time PCR.Results:1.The concentration of VCAM-1 in control group,good collateral circulation group and poor collateral circulation group were 155.36 ± 85.85,117.51 ±62.37,113.89 ± 56.1?g / L,The plasma concentration of VCAM-1 in the collateral circulation group was significantly lower than that in the control group(p <0.05),and the difference was statistically significant.However,plasma VCAM-1 levels did not differ significantly between the good CCC and poor CCC groups.2.The expression level of micro RNA-155 in the plasma of the collateral circulation group was 11.5 times higher than that in the healthy control group.The good group was 1.8 times of the healthy control group,The expression level of micro RNA-155 in the poor group was significantly different from that in the good group and the control group(p <0.05;p <0.01),the difference was statistically significant.However,plasma micro RNA-155 levels did not differ significantly between the good coronary collateral circulation group and controls.3.The expression level of micro RNA-155 in plasma of the experimental group was negatively correlated with Rentrop classification(R =-0.309,p <0.01),however,no correlation was observed between VCAM-1 levels and Rentrop grade or micro RNA-155.4.the AUC for micro RNA-155 was 0.668(95% confidence interval(CI):0.534–0.800;p = 0.024)and the optimal cut off value was 2.07 with sensitivity and specificity 56.1%,70.8% respectively.The AUC for VCAM-1 was 0.624(95% CI:0.497–0.772;p= 0.096)and the optimal cut off value of was 100.69 with sensitivity and specificity 58.5%,70.8% respectively.Conclsion:1.Micro RNA-155 was significantly higher in the poor CCC group,suggesting that it may inhibit the formation of coronary artery collateral vessels.2.Micro RNA-155 has strong specificity and sensitivity in predicting coronary collateral circulation,and can be used as an effective biomarker to evaluate collateral circulation.
