Background and Objectives:Pigmented villonodular synovitis is a group of unknown chronic synovial tumor-like proliferative lesions,for the characteristics is synovial hyperplasia,reddish brown villous nodular and calming hemosiderin,is a kind of benign and malignant proliferative disease.It can erosion joints,involve joints and bursa,the knee is the most susceptible of the joints.The disease occurs in 21 to 45 years old.Originally,it was considered a benign tumor,and support the idea that there are still some scholars,it is still controversial.Symptoms of knee joint PVNS mainly for joint swelling associated with pain,restricted joint flexion activities,flexion activities appear elastic ring even lock symptoms,severe cases can have quadriceps disuse,joint damage.A few PVNS limitations in growth,manifested in the joint cavity scattered in the mass,knee joint less diffuse swelling,but can have lock symptoms.The disease can seriously affect the patients quality of life.The clinical data of 36 patients with knee lesion from May 2012 to Sep 2016 were analyzed retrospectively,and the outcome of arthroscopic synovectomy for knee lesion was studied further.MethodsThe cliniacal data of 36 patients with pigmented villonodular synovitis who were performed with arthroscopic synovectomy for knee lesion from May 2012 to Sep 2016 of the first Affiliated Hospital of Zhengzhou University were analyzed retrospectively.It must included age,gender,present history,past history,personal history,family history and other datas.All the patients were followed up from 8~25 months postoperatively,the average 15 months.The Lysholm scoring system was used to score for knee function before operations and during the last follow-up,excellent(93-100),good(84-92),fair(65-83),poor(0-64).The Ogilvie-Harris scoring system was used to score for knee function before operations and during the last follow-up,excellent(10-12),good(7-9),fair(4-6),poor(0-3).The measurement data are expressed as mean ± standard deviation.Using SPSS18.0 statistical software for preoperative and postoperative knee function score matching t test,take the standard level α=<0.05,if P < 0.05,the difference is statistically significant.ResultsThe Lysholm score from preoperative 57.47±9.18 points increased to postoperative 95.67±7.45 points(P < 0.05).Excellent efficacy in 24(66.7%),good in 9(25%),fair in 2(5.5%),and poor in 1(2.8%).The Ogilvie-Harris score from preoperative 6.91±1.68 points increased to postoperative 11.17±0.52 points(P < 0.05).Excellent efficacy in 23(63.9%),good in 10(27.8%),fair in 2(5.5%),and poor in 1(2.8%),perfect of 91.7%.ConclusionsArthroscopic synovectomy can significantly relieve the symptoms of knee lesions,and advantages such as small incision,complete debridement,fewer complications and lower recurrence rate.It has definite clinical effect and rapid recovery.
