Study On Methods Of Preventing Deep Venous Thrombosis In The Lower Extremity By Applying Air Pressure Therapy Apparatus

Objects By means of observing different days of applying air pressure therapy apparatus during postoperative nursing after laparoscopic total hysterectomy,the aim of this study was to explore its preventive effect on deep venous thrombosis in the lower extremity and detect the effect on hemagglutination index and acquire the appropriate treatment time of using air pressure therapy apparatus.Methods A total of 104 patients who were treated in a provincial hospital of Henan province from June 2016 to December 2016,undergoing laparoscopic hysterectomy under general anesthesia were enrolled.They were randomly divided into test group(n = 52)and control group(n = 52).The patients in the test group were treated with air pressure therapy apparatus for 5 days on the basis of routine nursing care,while the patients in control group were treated for 2 days with air pressure therapy apparatus and routine nursing care.Blood coagulation index was measured by routine method before surgery and on the first,third and fifth day after surgery,in the meantime double lower extremities vascular doppler ultrasonography was used to confirm whether there is thrombogenesis.Processing data with SPSS 21.0 software,using (?)±s expressing numerical variable data,adopting t testing,ANOVA for repeated measurement,?2 testing on categorical variable data.? =0.05.Results 1.Base Data There is no statistical significance about the difference in age,body mass index,operation time,intraoperative blood loss and preoperative hemagglutination index between the test group and control group before treatment(P>0.05).2.Incidence of Thrombosis and Thrombosis Omen Related Symptoms The case of deep venous thrombosis in the lower extremity after laparoscopic total hysterectomy was none of all 104 patients.There were 1 cases of thrombosis omen related symptoms in test group and 3 cases in control group.There is no statistical significance about the incidence rate of the two groups(p>0.05).3.Blood Coagulation Index Prothrombin time(PT)in test group on prior day before operation,the first,third and fifth day after operation separately are 11.19±0.28s?11.13±0.33s?10.83±0.31s?10.65±0.26s;PT in control group separately are 11.12±0.30s?11.11±0.31s?10.70±0.31 s?10.62±0.16 s.(F interblock=6.125,P=0.015;F time=76.317,P<0.001;F interaction=0.662,P=0.5).Both groups were rapidly shrinking.Changes of the activated partial thromboplastin time(APTT)are also rapidly decreasing,the same as changes of PT.D-dimer in test group are separately 0.085±0.036 mg/L?0.185±0.036 mg/L?0.339±0.049 mg/L?0.346±0.050 mg/L.D-dimer in test group are separately 0.075±0.048 mg/L?0.179±0.054 mg/L?0.371±0.046 mg/L ? 0.523±0.043 mg/L(F interblock=104.321 P<0.001;F time=1344.393 P<0.001;F interaction=110.22 P<0.001).Both groups were rapidly increasing.Changes of the Fibrinogen(FIB)are also rapidly increasing,the same as changes of D-dimer.4.Comparison between groups On the first day after operation,FIB 272.21±11.77 mg/DL in test group is lower than 278.98±16.66 mg/DL in control group(t=2.393,P=0.019).On the third day,PT 10.83±0.31 s in test group is higher than 10.70±0.31 s in control group(t=2.076,P=0.040);D-dimer 0.346±0.050 mg/L in test group is lower than 0.371±0.045 mg/L in control group(t=2.650,P=0.009).On the third day,There is statistical significance about difference between two groups with only D-dimer in test group lower than the other(p<0.001).Conclusions 1.There was no case of thrombosis among patients applied with air pressure therapy apparatus for 2 days and 5 days during postoperative nursing after laparoscopic total hysterectomy.And Incidence of thrombosis omen related symptoms is similar between two groups.Therefore,we should continue to adopt current therapy time after operation for 2 days.2.PT and APTT on the first,third and fifth day after laparoscopic total hysterectomy are both shorter than that before operation,meanwhile,FIB and D-dimer are both increasing with relatively high condensation state and the risk occurrence of deep venous thrombosis in the lower extremity is increased.3.Prolonging treatment time of air pressure therapy apparatus has a certain effect on Blood Coagulation Index after operation.
