The Etiology And Treatment Of Peri-procedural Ischemia During Coiling For Intracranial Aneurysms With 48 Cases Report

ObjectIschemic complications of single center intracranial aneurysm embolization was retrospective analysised to summarize the causes of complicatioins and explore effective prevention methods.MethodsThe clinical data of 48 patients who had ischemic complications from January 2011 to March 2015 in the Department of Interventional Radiology,the First Affiliated Hospital of Zhengzhou University were collected while 1106 patients received aneurysm embolization treatment in the same period.The data was analysised to summarize the causes of complicatioins and explore effective prevention methods.Outcomes48 cases consist of 17 males and 31 females.Ages ranges from 36 to 77 with an average of 57.1±9.5 years old.The aneurysm size of these cases varies from 2.4 to 13.0mm with an average of 5.6±2.6mm.37 of them suffer subarachnoid hemorrhage that 20 cases are Hunt-Hess grade?,7 cases are Hunt-Hess grade ?,6 cases are Hunt-Hess grade ? and 4 cases are Hunt-Hess grade ?.The reasons of ischemic compications is vasospasm in 19 cases,thrombosis in 16 cases,cerebral artery occlusion in 6 cases,intraoperative hypotension in 4 cases and encephaledema in 3 cases.The treatment include anticoagulation,anti-platelet aggregation,anti-convulsions,boost,expansion and endovascular treatment.38 patients(70.8%)had a mRS score ?2 and 14 patients(38.2%)had a mRS score> 3.ConclusionsThe reasons of ischemic compications is vasospasm,cerebral artery thrombosis,cerebral artery occlusion,intraoperative hypotension and encephaledema.The treatment include anticoagulation,anti-platelet aggregation,anti-convulsions,boost,expansion and endovascular treatment.The reasons of ischemic compications occurring should be particularly analysised to solve it and prevent the the occurrence of ischemic complications.
