Objective To investigate the effect and safety of small dose of dexmedetomidine(DEX)combined with propofol application in colonoscopy,that provide the best clinical anesthesia method for painless colonoscopy patients,reduce patients in pain,making sure the safety in the process.Methods In this study,a randomized,parallel controlled study,186 cases of painless colonoscopy patients selected from our hospital were randomly divided into two groups,group I and II composite group,62 cases in each group,using the order encoding,opaque completely hidden way sealed envelopes,the content of this study recruited clinicians unaware of eligible patients,patients that meet the inclusion criteria and signed informed consent to receive the sealed envelope,opened the envelope and follow the prompts to anesthetic intervention on patients.The patients with simple group for propofol anesthesia,patients were given intravenous injection of propofol 1 ~ 1.5mg/kg;composite group I of fentanyl combined with propofol fentanyl anesthesia,0.5 ~ 1.0 ? g/kg,1 ~ 1.5mg/kg and intravenous propofol compound;group II,dexmedetomidine combined with propofol and 10 min in intravenous infusion of dexmedetomidine intravenous 0.5?g/kg injection of propofol 1 ~ 1.5mg/kg.In two groups after loss of eyelash reflex colonoscopy,body movement during surgery,intermittent propofol 0.5 ~ 1mg/kg.Recorded HR,MAP,RR and SPO2 before administration(T0),fiber colonoscopy(T1),a song(T2),spleen liver song(T3)and at the end of the examination(T4);record the onset time,recovery time(check until the end of time,with open eyes)(time from the hospital in awake patients to leave the examination room time),intraoperative dexmedetomidine and propofol dosage were recorded,and the incidence of body movement;the pain of patients after treatment were observed after 30 min analysis,the effects of dexmedetomidine and propofol for clinical efficacy and safety of painless colonoscopy.Result 1.There was no significant difference between the three groups in gender,age,ASA score,body weight and test time(P > 0.05)2.Three groups of patients with T0 time period of HR,MAP,RR and Sp O2 had no significant difference between the monitoring results(P > 0.05);according to monitoring results of HR,after administration of three groups of patients were reduced to different degrees,compared with the same group T0 has statistically significant difference(P < 0.05);the pure group of patients after administration to check the end of the HR more obvious changes,the simple group HR has significant difference with the composite group II in T2 ~ T4 time monitoring values(P < 0.05);RR monitoring results in changes in RR after administration of three groups were more obvious,T1 ~ T4 when There was significant statistical difference between the two groups(P < 0.05),and the difference was statistically significant between the T3 group and the group T0(P < 0.05)3.Compound II group and compound group onset time,recovery time after administration,the hospital stay time and the dosage of propofol four were significantly better than the simple group,the difference(P < 0.05);and the composite group II in four aspects are better than that of compound I group,the difference(P < 0.05).4.The simple group,the group I and group II,respectively,the incidence of body movement were 51.61%,35.48% and 11.29%,the difference between the three groups was statistically significant(P < 0.05)5.The excellent and good rate of analgesic effect in group I,group I and group II were 61.29%,72.58% and 90.32%,respectively.The difference between the three groups was statistically significant(P < 0.05)6.Simple group of adverse reaction cases in which the number of glossoptosis,significantly decreased blood pressure,respiratory depression,restlessness,dizziness and adverse reaction incidence was significantly higher than that of compound II group,difference(P < 0.05);simple group and complex group had significant differences in the incidence of respiratory depression(P < 0.05);group I and group II composite compound in agitation,adverse reactions occurred in cases of dizziness have significant difference(P < 0.05).Conclusion 1.Fentanyl,dexmedetomidine combined with propofol can reduce the dosage of propofol anesthesia in painless colonoscopy in clinic,dexmedetomidine is better.2.A small dose of dexmedetomidine combined with propofol in painless colonoscopy in clinic with rapid onset,rapid recovery,less adverse reaction.
