ObjectiveTo explore the relationship between the bacteriology of chronic periodontitis and ischemic stroke from the gene level and provide reference for the prevention and treatment of ischemic stroke patients with chronic periodontitis,by detecting the distribution of Porphyromonas gingivalis FimA genotype in patients with ischemic stroke and periodontitis with PCR method?Methods1.From November 2015 to November 2016,63 patients with ischemic stroke patients with chronic periodontitisin in department of Neurology were selected.During the same period,62chronic periodontitis were selected from the Dental Department.All subjects aged 35-75years old,all agreed to participate in the experiment and filled in the informed consent.Divide they into:schemic stroke with chronic periodontitis group(experimental group):63cases,35men and 28women,mild periodontitis in 6 people,moderate in 30,severe in 27.Chronic periodontitis group(control group):62cases,33men and 29 women,mild periodontitis in 7 people,moderate in 28,severe in 27.2(1)General data was collected in patients,detailed record of the patient's age,gender,etc(2)Blooding,Depth Probing,Attachment lossof all subjects in the mouth full dentition six loci(lip(buccal),tongue nearly,central and far)periodontal status were examined by the same professional dentist.(3)The full mouth of patients were divided into 4 quadrant,selectthe deepest per:iodontal pocket of each quadrant to collectthe plaque samples,and the samples were frozen and stored at-70?.(4)Bacterial genomic DNA was extracted by immunomagnetic beads method Extraction Kit.(5)Polymerase chain reaction(PCR)method was used to detect Pg and the FimA genotype of Pg positive samples.The amplified products were observed by electrophoresis.(6)The spss22.0 software package was used to analyze the data.Result1.Compared with chronic periodontitis group(experimental group)and chronic periodontitis group(control group),there was no significant difference in age,sex,and other general data(P>0.05).2.There was no significant difference between the two groups in Probing depth,PD),attachment loss(Attachment loss,AL)and detection of bleeding(Blooding of probing,BOP)(P>0.05).3.Detection of P.g in the experimental group was 79.37%greater than 66.13%in the control group,but the difference was not statistically significant(x2=2.77,p=0.096>0.05);P.g can be detected in different degree of inflammation in two groups,in the experimental group,the detection rate of P.g in severe periodontitis was 92.59%higher than that of mild 33.33%and moderate76.67%,and the difference was statistically significant(x2=10.78,p=0.005<0.05);the control group in different degree of periodontal inflammation between the detection rate of P.g had no significant difference(P>0.05);4.Both the experimental group and the control group can detect a variety of P.g FimA genotypes,in which the highest detection rate was type II FimA genotypes,followed by type IV and V were not detected;In the experimental group,the total detection rate(62.00%)of ? was significantly higher than that of the control group(39.02%),and the difference was significant(P<0.05);There was no significant difference between the two groups in the detection rate of the other five types of FimA(P>0.05);P.g detected in the two groups were mostly single type,a small number of patients is the joint detection of multi FimA.Conclusion1.The detection rate of P.g in ischemic stroke patients with severe periodontitis with was higher than that in mild and moderate patients.2.FimA gene polymorphism was found in ischemic stroke and periodontitis:FimA ? genotype is the main genotype.3.Ischemic stroke with periodontitis were more likely to be infected with P.g FimA ?,which was more likely to be infected with inflammation.
